【摘要】:目的:随着经济发展和物质文化需求的提高,人们更加视身体健康和形象良好,越来越多的人不仅仅希望有一个健康的身体,还希望有一个完美的微笑。牙齿的颜色在微笑面容中是一个很重要的部分。牙齿颜色过深,牙齿着色,牙齿变色,以及发育性、药物性牙齿颜色不良在临床中十分常见,每个年龄组人群均可发生。根据变色原因的不同分为:外源性着色、内源性着色,临床中有很多方法改善牙齿着色。其中,活髓牙美白分为三种方法:诊室内美白、家庭美白以及非处方美白药物。诊室内美白是在牙面涂布含有高浓度过氧化氢(15%~40%)的美白剂,一次治疗后可取得较为明显的效果。这种牙齿美白方法具有美白效果良好、安全性高和对牙齿无不可逆性损伤等优点,因此被广泛应用于临床。本研究旨在评价两种临床中广泛应用的美白药物,Beyond冷光美白与OpalescenceBoost美白(即常用的普洋冷光美白和皓齿美白),评价牙齿的美白效果及美白持久性,以及在美白过程中和完成后,牙齿敏感性有无差异。材料方法:根据实验目的制定严格的志愿者纳入标准,并按照此标准筛选志愿者。将18名受试者纳入本实验研究,将每一名志愿者上下领从左侧第一前磨牙到右侧第一前磨牙选定为实验牙齿,并从中线处分为左右两侧,两侧随机分别使用Beyond冷光美白系统与OpalescenceBoost美白系统。在进行美白前进行基线确定:使用Vita Easyshade电脑比色仪、和微距相机采集相片并使用软件进行色彩分析,测量并记录每位志愿者16颗受试牙中央部位a*值、b值*、L*值,根据公式计算牙齿美白前后的△E值。比较Beyond冷光美白系统与OpalescenceBoost美白系统是否具有确切的美白效果、二者美白效果的差异、美白效果的持久性。同时调查志愿者美白术中及美白后牙齿敏感情况,对美白效果的满意度。最后对研究数据进行统计学处理(SPSS22.0),并分析得出结论。结果:1、Beyond冷光美白系统与OpalescenceBoost美白系统在美白后牙齿的明度(L*)、白度(b*)具有明显提高,同时△E≥2.75,p值均小于0.05,说明二者具有确切的美白效果。2、Beyond冷光美白系统与OpalescenceBoost美白系统在美白后、48小时、一周、一月、三月各个时间点内,美白效果没有明显差异,p值均大于等于0.05,说明二者美白效果随时间推移没有表现出差异。3、Beyond冷光美白系统与OpalescenceBoost美白系统在美白后、48小时、1周、一月、三月美白效果都没有明显变化,P0.05,表明二者美白效果持续性较好。4、83.3%的志愿者出现可忍受牙齿敏感,并在48小时内敏感可大部分自愈。5、88.9%志愿者对于美白效果满意,44.4%的志愿者在美白完成时能主观观察到实验两侧牙齿有轻微颜色差异(Beyond冷光美白系统略白于OpalescenceBoost美白系统),但此轻微差异会在48小时内消失。结论:1、诊室美白后,牙齿美白效果明显,明度变高,黄度减少;美白效果持久性高。2、两种美白产品在3个月内的美白效果均表现为:确切有效,持续可靠,左右两侧颜色对称,无明显差异。3、两种美白剂引起的牙齿轻微敏感可以被接受,Beyond美白剂与OpalescenceBoost美白剂同一个体敏感性无明显差异,敏感性高低取决于受试者个体因素。临床医师在选择美白剂时,应先充分考虑患者的个体的敏感性。4、综合考虑两种美白产品的时间和经济成本,可以为患者选择出更适合的美白产品。
[Abstract]:Objective: with the development of economy and improvement of material and cultural needs, people more visual health and good image, more and more people just want to have a healthy body, but also want to have a perfect smile. The color of the teeth is a very important part in the smiling face. The tooth color is too deep, teeth coloring, tooth discoloration, and the development of drug-induced adverse, tooth color is very common in clinical practice, each age group can occur. According to the different reasons for discoloration: exogenous endogenous coloring, coloring, there are many ways to improve clinical stain teeth. The teeth whitening is divided into three methods: consulting room in whitening, whitening family and non prescription drugs. Consulting room whitening whitening is containing a high concentration of hydrogen peroxide in the tooth surface coating (15% ~ 40%) whitening agent, after one treatment can obtain obvious effect. This kind of tooth Whitening whitening method has good effect, high safety and no teeth reversible injury etc, so it is widely used in clinic. The purpose of this study is to evaluate the two kinds of whitening agents are widely used in clinical medicine, Beyond whitening whitening and OpalescenceBoost (that is commonly used in whitening teeth whitening and Pu Yang), evaluation of tooth whitening the effect of whitening and persistence, and the completion of the whitening process after neutralization, there is no difference of tooth sensitivity. Materials and methods: according to the strict criteria of volunteers, and in accordance with the standard screening volunteers. Clinicians in the selection of whitening agent, should fully consider the sensitivity of.4 of individual patients, considering two kinds of whitening products and time the economic cost, can choose more suitable whitening products for patients.
