成年安氏Ⅱ类1分类患者颌骨位置改变对软组织侧貌的影响

【摘要】:研究目的: 通过SN重叠与上下颌重叠治疗前后牙体组织位置变化的比较来探讨成年安氏II类1分类患者正畸治疗后颌骨位置的变化以及其位置变化对软组织侧貌的影响。并进一步探讨国人软硬组织变化的相关性,为临床矫治计划的制定和矫治效果的预测提供参考依据。 材料方法: 1.将选取的90例成年安氏II类1分类患者治疗前后的X线头颅侧位片分别进行前颅底S-N重叠,上颌重叠和下颌重叠,通过比较不同重叠情况下的牙体组织位置变化来探讨颌骨的位置变化; 2.获取患者治疗前后X线头颅侧位片,,经描绘、测量以及统计学分析,综合实验一的结果,探讨颅底重叠和下颌骨重叠时牙体组织位置改变与软组织改变的相关性差异比较以及下颌骨的旋转对软组织侧貌的影响。 结果: 1. SN重叠与上颌重叠时上切牙在矢状和垂直方向上的位置变化比较没有统计学差异。SN重叠与下颌重叠时下切牙在矢状方向上位置变化比较没有统计学差异但在垂直方向上的位置变化比较有统计学意义。上颌骨参照线在治疗后的改变没有统计学差异而下颌骨参照线在治疗后的改变有统计学差异。 2.下颌重叠时下切牙在垂直方向上的位置变化与软组织侧貌改变没有相关关系,而颅底重叠时下切牙在垂直方向上的改变与上唇内收成负相关关系。下颌的旋转参与影响Z角的变化。 结论: 1.上颌骨在矫治后没有发生旋转改变。而下颌骨发生了顺时针旋转的改变,顺时针旋转0.47°。 2.下颌重叠与SN重叠时下切牙位置改变与软组织侧貌改变的相关性比较是存在差异的。下颌的旋转角度会直接对软组织侧貌产生影响,同时下颌的旋转还可能通过改变下切牙在水平和垂直方向上的位置来间接影响软组织侧貌。
[Abstract]:Objectives of the study:. By comparing the changes of tooth tissue position between SN overlap and upper mandibular overlap before and after orthodontic treatment, the changes of jaw position in adult patients with Class II class 1 orthodontic treatment and their effects on soft tissue profile were studied. And further explore the correlation between the changes of Chinese soft and hard tissue, To provide a reference for the establishment of clinical treatment plan and the prediction of therapeutic effect. Material methods:. 1. S-N overlap, maxillary overlap and mandibular overlap of anterior skull base, maxillary overlap and mandibular overlap were performed in 90 adult patients with classification II 1 before and after treatment. By comparing the position changes of tooth tissue in different overlaps, the changes of jaw position were discussed. 2. Obtain the X-ray lateral radiographs of patients before and after treatment, and synthesize the results of experiment one by description, measurement and statistical analysis. To explore the difference between the position change of tooth tissue and the change of soft tissue in the overlap of skull base and mandible, and the effect of mandibular rotation on soft tissue profile. Results:. 1. There was no significant difference in sagittal and vertical position of incisors between SN overlap and maxillary overlap. There was no statistical difference between SN overlap and mandibular overlap in sagittal direction, but there was no significant difference in sagittal position between upper incisors and mandibular overlaps. There was no statistical difference in the changes of maxillary reference line after treatment, but there was statistical difference in mandibular reference line after treatment. 2. There was no correlation between the position change of incisors in vertical direction and the change of soft tissue profile. The vertical change of incisor is negatively related to the upper lip harvest, and the rotation of mandible is involved in the change of Z angle. Conclusion:. 1. The maxillary bone did not change in rotation after orthodontic treatment, but the mandible rotated clockwise, with a clockwise rotation of 0.47 掳. 2. The correlation between the position change of incisor and the lateral appearance of soft tissue is different when the mandibular overlap and SN overlap, and the rotation angle of the mandible has direct influence on the soft tissue profile. At the same time, mandibular rotation may indirectly affect soft tissue profile by changing the position of lower incisors in horizontal and vertical directions.
