自然牙根和种植体联合支持的磁性附着体覆盖义齿应力分析

【摘要】:目的:研究自然牙根和种植体联合支持的磁固位全口覆盖义齿不同受力状态下支持骨的应力分布,为临床修复设计提供参考。方法:采用CT技术、计算机图像处理系统及Solidworks有限元软件,根据自然牙根与种植体不同位置,放置3对磁性附着体,建立3组下颌覆盖义齿的三维有限元模型,其中模型Ⅰ种植体在磨牙区,模型Ⅱ种植体在两侧,模型Ⅲ种植体在一侧。在不同的应力加载下对3组模型中自然牙和种植体颈部骨应力进行计算,对比和分析。结果:模型Ⅱ斜向加载下后牙区种植体周围骨应力较模型Ⅲ增加明显。斜向加载较垂直加载的支持骨应力均有增加,其中模型Ⅰ中的后牙区种植体周围骨应力增加约30%,模型Ⅱ增加约43%,模型Ⅲ增加约55%。结论:当口内仅余留1个自然牙根时,至少应增加2个种植体作为基牙,且这2个种植体应位于牙弓的两侧,即一侧前牙区和另一侧的后牙区。磁性附着体的结构比其他类型的附着体能更好地防止侧向力对基牙的损害。
[Abstract]:Objective: To study the stress distribution of the supported bone under the different stress state of the magnetic retainer full denture supported by natural tooth root and implant, and to provide reference for the clinical repair design. Methods: using CT technology, computer image processing system and Solidworks finite element software, 3 pairs of magnetic attachment were placed according to the different positions of the natural root and the implant. The three-dimensional finite element model of 3 groups of mandibular overdentures was established in the body, in which model I implant was in the molar area, model II implant was on both sides, model III implant was on one side. Under different stress loading, the bone stress of the natural teeth and implant neck in the 3 models was calculated, compared and analyzed. Results: Model II oblique loading under the posterior teeth. The bone stress around the implant was more obvious than that of the model III. The support bone stress in the oblique loading was increased by 30%, the model II increased by about 43%, and the model III increased by about 55%. conclusion: at least 2 implants should be increased when only 1 natural roots were left in the mouth. For the base teeth, the 2 implants should be located on both sides of the dental arch, that is, one side of the anterior teeth and the other side of the posterior teeth. The structure of the magnetic attachment is better than other types of attachments to prevent the lateral force from damaging the base teeth.
