冠心病患者牙齿缺失数目与颈动脉病变相关性的临床研究

【摘要】：目的:通过对冠心病住院患者进行问卷调查和口腔检查,初步探讨牙周炎与动脉粥样硬化之间的相关性。方法:选择经病史和冠状动脉造影确诊为冠心病的患者,详细记录其人口特征、血压、糖尿病病史、心血管病史、血脂系列结果以及口腔情况。颈动脉超声检测颈动脉内中膜厚度(carotid artery intima-media thickness,CIMT)及斑块情况。结果:非条件Logistic回归分析结果显示缺牙数目与斑块发生存在正相关(OR=1.298,95%CI:1.100-1.531,P=0.002);简单线性回归分析结果显示缺牙数目与CIMT存在线性关系,回归系数b=0.041。缺牙数目与斑块直径之间存在相关关系,相关系数rs=0.519,P0.001。缺牙数目与血脂各项指标之间无统计学意义。结论:对于本研究人群(冠心病住院患者),随着缺牙数目的增加,颈动脉斑块的发生率增加,颈动脉内中膜厚度及斑块的大小也增加,这表明牙周炎导致的牙齿缺失可能是动脉粥样硬化疾病的一个重要的风险指标。
[Abstract]:Objective: To investigate the correlation between periodontitis and atherosclerosis by questionnaire and oral examination of the patients in the patients with coronary heart disease. Methods: select patients with coronary heart disease diagnosed by medical history and coronary angiography, and record their demographic characteristics, blood pressure, history of glucose and urine, cardiovascular history, blood lipid series and mouth. Cavities. Carotid artery ultrasound (carotid artery intima-media thickness, CIMT) and plaque were detected by carotid artery ultrasound. Results: the results of non conditional Logistic regression analysis showed that there was a positive correlation between the number of teeth missing and the occurrence of plaque (OR=1.298,95%CI:1.100-1.531, P=0.002), and the results of simple linear regression analysis showed the number of teeth missing and CIMT. There is a linear relationship. There is a correlation between the number of teeth missing and the diameter of plaque b=0.041.. There is no statistical significance between the correlation coefficient rs=0.519, the number of P0.001. teeth missing and the indexes of blood lipid. Conclusion: the incidence of carotid plaque increases with the increase of the number of tooth deficiency in this study group. The intima media thickness and plaque size also increased, indicating that tooth loss caused by periodontitis may be an important risk indicator for atherosclerotic disease.
