基于有限元的儿童牙齿重新排列的诱导方案研究

【摘要】:儿童隐形诱导器是近几年才出现的一种引导儿童牙齿重新排列的新装置,以其舒适性、美观性、便捷性以及疗效可预见性受到众多儿童患者的青睐,迅速的在正畸界得到应用和推广。而儿童隐形诱导器的独特作用方式使得医生对诱导过程中产生何种形式的力系以及怎样的作用方式缺乏认识,这在很大程度上制约了儿童隐形诱导技术的全面推广。进一步研究诱导器作用下的牙颌组织力学特性以及作用力系,可以为医生制定、修改以及优化诱导方案提供指导。本文结合临床的儿童诱导过程,利用数字化技术、3D打印技术以及有限元仿真技术对儿童整个动态诱导过程做了分析和研究。首先,利用数字化技术快速准确的建立儿童牙颌三维模型,根据生物学上骨重建的原理,提出了骨重建力学仿真的数值模型,得到牙周膜应变与牙齿移动速度的关系。通过有限元分析得到诱导器作用下的牙周膜应变情况,从而建立一个连续诱导过程的有限元模型,通过分析连续两次更换诱导器的儿童切牙整体移动的动态诱导过程,得到诱导过程中诱导器的作用力系以及牙颌组织的生物力学状况,补充和完善了医生对诱导过程中作用力系和作用方式的认识。其次,研究了个体差异在整个诱导过程中对作用力系和作用方式的影响,个体差异对作用力系的影响有限,主要反映在对力偶的改变上。最后,分析了儿童整体移动诱导过程中对其它牙齿的影响,发现各个牙齿都承担起支抗牙的作用,支抗力由重新排列牙齿的邻牙向周围传递。
[Abstract]:The child stealth inducer is a new device to guide the rearrangement of children's teeth in recent years. It is favored by many children because of its comfort, beauty, convenience and predictability of curative effect. It is rapidly applied and popularized in orthodontic field. However, the unique role of the child stealth inducer makes doctors lack the understanding of what form of force system and how to act in the induction process, which to a great extent restricts the comprehensive popularization of the stealth induction technology of children. The further study of the mechanical properties of dental and maxillary tissues and the force system under the action of the inducer can provide guidance for doctors to formulate, modify and optimize the induction scheme. In this paper, the whole dynamic induction process of children is analyzed and studied by using digital 3D printing technology and finite element simulation technology combined with the clinical induction process of children. Firstly, the three-dimensional model of dental and maxilla of children was established quickly and accurately by digital technology. According to the principle of bone reconstruction in biology, the numerical model of bone reconstruction mechanics simulation was put forward, and the relationship between the strain of periodontal membrane and the speed of tooth movement was obtained. The strain of periodontal membrane under the action of inducer is obtained by finite element analysis, and a finite element model of continuous induction process is established, and the dynamic induction process of the whole movement of incisor in children with continuous twice replacement inducer is analyzed. The force system of the inducer and the biomechanical condition of the dental and maxillary tissue were obtained, which supplemented and improved the doctors' understanding of the force system and the mode of action during the induction process. Secondly, the influence of individual difference on the force system and the mode of action in the whole induction process is studied. The effect of individual difference on the force system is limited, which is mainly reflected in the change of the couple. Finally, the influence of children on other teeth during the whole movement induction is analyzed. It is found that each tooth plays the role of Anchorage, and the branch resistance is transferred from the adjacent teeth of the rearranged teeth to the surrounding teeth.
