【摘要】:目的: 口腔鳞状细胞癌(oral squamous cell carcinima, OSCC)恶性程度高,浸润性强,且易发生淋巴结及远处脏器转移。目前针对OSCC的研究较多的集中于细胞凋亡和增殖的平衡机制方面,Livin和Caspase-3是细胞凋亡和增殖过程中重要的生物学指标。本实验通过探讨凋亡抑制蛋白(IAP) Livin和半胱氨酸天冬氨酸蛋白酶-3(Caspase-3)在口腔鳞状细胞癌(OSCC)中的表达及其相关性,从分子学水平研究Livin和Caspase-3mRNA在口腔鳞状细胞癌(OSCC)和正常口腔黏膜组织(NOT)中的表达,分析二者在口腔鳞状细胞癌中表达的相关性,为对患者诊断及监测、评估术后复发、转移的风险程度提供参考,以辅助临床工作。 方法: 本研究应用逆转录-聚合酶链反应(reverse transcription polymerase chain reaction, RT-PCR)方法,选取2011年10月~2013年2月在安徽医科大学第一附属医院口腔颌面外科就诊,并均经组织病理检查确诊为OSCC患者新鲜组织标本45例,所有OSCC组患者均为首次确诊,术前均未行放、化疗或其他抗肿瘤治疗,均无远端脏器的转移,无其他恶性肿瘤史,无家族肿瘤史。另选11例患者为阴性对照组(NOT组),其中7例取自阻生齿拔除患者,4例取自牙槽嵴修整患者。所有标本均于术中取材后立即置于液氮中,然后移至-80℃超低温冰箱中贮存备用。 结果: 1.Livin mRNA和Caspase-3mRNA在OSCC组织中的相对含量均与组织分化程度、淋巴结转移和临床分期均有相关性;但与患者的年龄、性别无关。 2.Livin和Caspase-3在45例OSCC和11例NOT中均有不同程度的表达:Caspase-3mRNA在OSCC组织中的阳性表达率明显低于NOT (P0.05);其在NOT中相对含量(0.193±0.031),高于OSCC(0.102±0.025)(P0.01)。Livin mRNA在OSCC组织中的阳性表达率明显高于NOT (P0.05);其表达量在OSCC组织中为(0.542±0.074),明显高于NOT (0.181±0.026)(P0.01). 3.根据Pearson列联系数相关分析, Livin mRNA和Caspase-3mRNA在OSCC组织中的表达呈相关关系(P0.01)。 结论: Livin和Caspase-3在OSCC中的表达结果分析显示,两者可能参与了OSCC的发生、发展的过程,联合检测Livin与Caspase-3可以为研究OSCC发病机制提供新的思路,为OSCC的临床分级、分期、评估患者预后提供数据支持。
[Abstract]:Objective: Oral squamous cell carcinima, OSCC) in oral squamous cell carcinoma is highly malignant, invasive and prone to lymph node and distant organ metastasis. At present, the researches on OSCC focus on the balance mechanism of apoptosis and proliferation. Livin and Caspase-3 are important biological indexes in the process of cell apoptosis and proliferation. In this study, we investigated the expression of Livin and caspase-3 in oral squamous cell carcinoma (OSCC) and their correlation with the expression of apoptosis-inhibiting protein (IAP) and cysteine aspartate proteinase-3 (Caspase-3) in oral squamous cell carcinoma (OSCC). To study the expression of Livin and Caspase-3mRNA in oral squamous cell carcinoma (OSCC) and normal oral mucosa tissue from molecular level, and to analyze the correlation between them in oral squamous cell carcinoma (OSCC) and normal oral mucosa. The degree of risk of transfer provides a reference to assist clinical work. Methods: In this study, reverse transcription polymerase chain reaction, RT-PCR method was used to select oral and maxillofacial surgery patients from October 2011 to February 2013 in the first affiliated Hospital of Anhui Medical University. 45 cases of fresh tissue specimens were confirmed by histopathological examination. All the patients in OSCC group were diagnosed for the first time. No radiotherapy, chemotherapy or other antitumor therapy, no metastasis of distal organs and no history of other malignant tumors were performed before operation. No family history of cancer. Another 11 cases were selected as negative control group, 7 cases were extracted from impacted teeth, 4 cases were from alveolar ridge dressing.
