计算机辅助技术设计下颌升支矢状劈开术同期行颏成形的临床应用

【摘要】:背景:我国部分下颌前突患者同时伴有颏突度不足为表现的颏后缩畸形,单纯行下颌升支矢状劈开术,术后面下部外形往往难以令人满意。 目的:探讨同期行下颌升支矢状劈开术及颏成形手术操作的必要性及可行性,研究计算机辅助技术设计同期手术的精确性和优越性。 方法:收集7例下颌前突伴发颏后缩畸形患者,应用计算机辅助技术设计下颌升支矢状劈开同期行颏成形的手术方案,并制作个性化导板,指导手术操作。通过比对术前设计和术后的头颅三维重建模型,评估设计的精准性;同时比对术后即刻与术后随访模型,评估疗效的稳定性。 结果:7例患者术后均取得满意的外形改善及咬牙合关系。统计学分析:术前和术后即刻存在明显统计学差异;术前设计和术后即刻无统计学差异;术后即刻与随访除SNB、Pog-NB余无统计学差异(P0.05)。 结论:计算机辅助技术能精确设计、模拟下颌升支矢状劈开同期行颏成形,提高手术精准度。 小结:本研究立足于临床,将计算机辅助技术和下颌前突伴颏后缩畸形的治疗紧密结合。在本研究中,作者利用计算机辅助技术的优势,精确设计下颌升支矢状劈开同期行颏成形的手术方案,并利用计算机辅助制造技术制作相应个性化导板,精准指导手术操作,节约手术时间,降低手术风险。术后外形及功能改善良好,术后随访疗效确切。
[Abstract]:Background: some patients with mandibular protrusion in our country are accompanied by mental retraction deformity with insufficient mental process. Sagittal splitting of the mandibular ramus is simply performed, and the shape of the lower part of the mandible is often unsatisfactory. Objective: to investigate the necessity and feasibility of simultaneous sagittal splitting of mandibular ramus and mental prosthesis, and to study the accuracy and superiority of computer aided design for simultaneous surgery. Methods: seven patients with mandibular protrusion accompanied with mental retraction deformity were collected. The surgical scheme of sagittal splitting of mandibular ramus was designed by computer aided technique, and the individualized guide plate was made to guide the operation. The accuracy of the design was evaluated by comparing the preoperative design with the post-operative 3D reconstruction model, and the stability of the therapeutic effect was evaluated by comparing the immediate postoperative and postoperative follow-up models. Results all 7 cases had satisfactory shape improvement and occlusal relationship after operation. Statistical analysis: there was significant statistical difference before and after operation; there was no statistical difference between preoperative design and postoperative immediate; and there was no statistical difference between immediate postoperative and follow-up except SNBN Pog-NB (P0.05). Conclusion: the computer aided technique can accurately design and simulate the sagittal splitting of mandibular ramus. Summary: this study is based on clinical, the treatment of mandibular protrusion with mental retraction deformity is closely combined with computer assisted technique. In this study, the author made use of the advantage of computer-aided technology to design the surgical scheme of sagittal splitting of mandibular ramus for simultaneous genioplasty, and made the corresponding individualized guide plate by using computer-aided manufacturing technology to accurately guide the operation. Save the operation time and reduce the risk of operation. The appearance and function were improved well after operation, and the follow-up effect was definite.
