CGF促进颌骨囊肿术后骨缺损修复的临床研究

【摘要】:目的本实验中,将自体浓缩生长因子(CGF)单独作为骨移植材料对颌骨缺损处进行填充。与空白组进行对比,在观察期内进行CBCT(口腔锥体束CT)的拍摄,利用缺损处的骨密度值来评价手术后骨腔内的骨质重建。材料和方法36例囊肿术后骨缺损最大直径在4.0cm以下,0.5cm以上的患者,按照填充方法的不同随机分为两组:18例患者在缺损处的骨腔内填入CGF凝胶作为CGF组,18例术后骨腔内不填入任何材料作为空白组。依据术中冰冻结果诊断为含牙囊肿的患者9例,根尖囊肿20例,鼻腭囊肿7例,上颌部囊肿17例,下颌部囊肿19例。囊肿直径在(0.8~3.8)cm之间,平均为(2.51±1.87)cm。男性19例,女性17例。年龄10~47岁,平均年龄为26.42岁。所有患者于术后1、2、4个月复诊并拍摄CBCT,使用Mimics Research 17.0图像分析软件对颌骨缺损区的骨密度值进行测量,使用SPSS17.0统计软件进行统计学分析。结果1、同一组不同观察时间在成骨效果上相比,CGF组和空白组之间有统计学差异。(P小于0.05)2、术后1月时,CGF组与空白组结果间无显著性差异(P大于0.05)术后2月时,CGF组与空白组结果间有显著性差异(P小于0.05)术后4月时,CGF组与空白组结果间有显著性差异(P小于0.05)结论本次实验研究结果支持了CGF的促成骨作用,并且可作为单一材料,应用于骨缺损区域的修复,加速新生骨的生成。在临床上,为患者缩短了治病疗程,减少了疾病愈合的痛苦。
[Abstract]:Objective in this experiment, autogenous concentrated growth factor (CGF) was used as bone graft material to fill the mandibular defect. Compared with the blank group, CBCT (oral pyramidal tract CT) was taken during the observation period. Bone mineral density (BMD) of the defect was used to evaluate the bone reconstruction in the bone cavity after operation. Materials and methods 36 patients with bone defect whose maximum diameter below 4.0cm were more than 0.5 cm after operation were analyzed. According to the different filling methods, two groups of 18 patients were randomly divided into two groups: 18 cases were treated with CGF gel in the bone cavity of the defect as the CGF group and 18 cases were treated as the blank group without any material filling in the bone cavity after operation. According to intraoperative frozen results, 9 cases were diagnosed as dental cysts, 20 cases as apical cysts, 7 cases as nasopalatine cysts, 17 cases as maxillary cysts, 19 cases as mandibular cysts. The mean diameter of cyst was (2.51 卤1.87) cm. There were 19 males and 17 females. The average age was 26.42 years. All the patients were followed up for 2 and 4 months after operation, and CBCTs were photographed. The bone mineral density was measured by Mimics Research 17.0 image analysis software, and SPSS 17.0 statistical software was used for statistical analysis. Results 1. There was significant difference in osteogenic effect between CGF group and blank group at different observation time (P < 0.05). 2. There was no significant difference between CGF group and blank group at 1 month after operation (P > 0.05) and between CGF group and blank group at 2 months after operation. There was significant difference between CGF group and blank group at 4 months after operation (P < 0.05). Conclusion the results of this experiment support the effect of CGF on bone formation. It can be used as a single material to repair bone defect and accelerate the formation of new bone. Clinically, the course of treatment is shortened and the pain of healing is reduced.
