肥大细胞干细胞因子在人慢性根尖周病中的表达

【摘要】：目的:观察干细胞因子(stem cell factor,SCF)在慢性根尖周病患者肥大细胞(mast cells,MCs)中的表达情况,探讨SCF阳性MCs在根尖周病发病机制及根尖周纤维组织的形成,尤其在根尖周囊肿纤维组织形成中的作用。方法:选择2011年7月~2013年6月期间在暨南大学荔湾口腔教学医院临床诊断为根尖囊肿或根尖肉芽肿并接受根尖刮治手术的病例30例,其中根尖肉芽肿15例,根尖囊肿15例。正常对照组20例,取自因正畸需要拔除的牙周健康的前磨牙。标本经10%甲醛固定48 h以上。组织标本经石蜡包埋、组织连续切片,HE染色,光学显微镜下观察其组织学改变;采用免疫组织化学法,观察各组标本组织中MCs数目及MCs脱颗粒情况;采用免疫荧光双染色法,在荧光显微镜下观察各组标本组织中tryptase-SCF双阳性MCs数目。结果:免疫组织化学染色结果显示,2组慢性根尖周病组织中MCs和脱颗粒MCs数量与正常对照组相比显著增多(P0.01);根尖囊肿组的MCs和脱颗粒MCs的数量均明显高于根尖肉芽肿组(P0.05)。免疫荧光双染色结果显示,2组慢性根尖周病组织中的tryptaseSCF双阳性MCs密度与正常对照组相比显著增多(P0.01);根尖囊肿组tryptase-SCF双阳性MCs密度明显高于根尖肉芽肿组(P0.05)。各组标本组织中免疫组织化学染色MCs密度与免疫荧光双染色MCs密度无显著差异(P0.05)。结论:Tryptase-SCF双阳性肥大细胞在慢性根尖周病的炎症损害,尤其在根尖囊肿的纤维组织形成中发挥重要作用。Tryptase-SCF双阳性肥大细胞与慢性根尖周病炎症损害的发生、进展及其持续存在有关。
[Abstract]:Objective: to observe the expression of stem cell factor (stem cell factor,SCF) in mast cells (mast cells,MCs) of patients with chronic periapical disease, and to investigate the pathogenesis of SCF positive MCs in periapical disease and the formation of periapical fibrous tissue. In particular, the role of periapical cysts in the formation of fibrous tissue. Methods: from July 2011 to June 2013, 30 cases of apical cyst or apical granuloma, including 15 cases of apical granuloma and 15 cases of apical cyst, were selected from Liwan Stomatological Teaching Hospital of Jinan University. 20 cases of normal control group were extracted from healthy premolars due to orthodontics. The specimens were fixed with 10% formaldehyde for more than 48 hours. Tissue specimens were embedded in paraffin wax, stained with HE and stained with HE, and their histological changes were observed under optical microscope. The number of MCs and MCs degranulation were observed by immunohistochemical method, and immunofluorescence double staining was used. The number of tryptase-SCF double positive MCs in tissues of each group was observed under fluorescence microscope. Results: the results of immunohistochemical staining showed that the number of MCs and degranulated MCs in the chronic periapical disease tissues of the two groups was significantly higher than that in the control group (P0.01), and the number of MCs and MCs in the apical cyst group was significantly higher than that in the apical granuloma group (P0.05). The results of double immunofluorescence staining showed that the density of tryptaseSCF double positive MCs in the tissue of chronic periapical disease was significantly higher than that in the control group (P0.01), and the density of tryptase-SCF double positive MCs in the apical cyst group was significantly higher than that in the apical granuloma group (P0.05). There was no significant difference in the density of immunohistochemical staining MCs and immunofluorescence double staining MCs in each group (P0.05). Conclusion the double positive mast cells play an important role in the inflammatory damage of chronic periapical disease, especially in the formation of fibrous tissue of the apical cyst. The occurrence of the inflammatory damage in chronic periapical disease and the occurrence of the double positive mast cells of Tryptase-SCF may play an important role in the pathogenesis of chronic periapical disease. Progress and its persistence are relevant.
