牙本质肩领形态与桩核类型对桩核冠修复后牙体抗折性能的影响

【摘要】:目的:研究在人工老化的条件下,不同形态的牙本质肩领与桩核系统对桩核冠修复后的牙体抗折性能的影响。方法:收集三个月内因正畸治疗拔除的下颌单根管第一前磨牙64颗,随机分成8组(n=8),各组离体牙预备成不同的牙体缺损形态,设计为不同的牙本质肩领及桩核类型。A1组:360°牙本质肩领+玻璃纤维桩树脂核。A2组:360°牙本质肩领+铸造钴铬合金桩核。B1组:180°颊侧牙本质肩领+玻璃纤维桩树脂核。B2组:180°颊侧牙本质肩领+铸造钴铬合金桩核。C1组:180°舌侧牙本质肩领+玻璃纤维树脂桩核。C2组:180°舌侧牙本质肩领+铸造钴铬合金桩核。D1:180°颊、舌侧牙本质肩领+玻璃纤维桩树脂核。D2:180°颊、舌侧牙本质肩领+铸造钴铬合金桩核。牙本质肩领高度均设为2mm,厚度设为(1.2±0.2)mm。桩核粘接就位后所有离体牙均按照全冠预备标准进行钴铬合金铸造金属全冠修复。将8组样本全部进行冷热循环(5℃-55℃)处理5000次。在电子万能试验机上以与牙长轴成150°于颊尖顶处加载,加载速度设置为1.0mm/min,直至试件任何一处出现断裂,对各组折裂强度进行方差分析、t检验统计分析;观察折裂模式的区别,并进行卡方检验(α=0.05)。结果:1各组试验牙齿的牙根长度、釉牙骨质界处近远中径、颊舌径均无统计学上的差异(P0.1)。2纤维桩树脂核组及铸造金属桩核组抗折强度最高值都出现在360°环形组,统计结果显示与其他组间存在明显差异(P0.05)。其余各组间未见明显差异。3铸造金属桩核组的抗折强度高于纤维桩树脂核组,但在保留舌侧肩领类型组的差异无意义,其余三组间差异均有统计学意义(P0.05)。4纤维桩核组可修复性折裂例数明显高于铸造金属桩核组。卡方检验显示差异具有统计学意义(P0.05)。结论:1牙本质肩领的完整性对桩核冠修复后牙体抗折性起着决定性作用,因此临床上应尽量多地保留牙本质肩领来提高修复后牙体的抗折性能,牙本质肩领位置形态对牙体抗折性能的影响较小。2铸造金属桩核抗折强度高,但易造成牙体的不可修复性根折,纤维桩核虽抗折强度较低,但能满足临床需求,且更有利于牙根的保护。
