上颌尖牙腭侧阻生邻近侧切牙冠根形态特征的锥形束CT研究

【摘要】:目的上颌尖牙腭侧阻生是口腔常见的阻生畸形,但其病因仍存在争议。文中旨在通过锥形束CT(Cone-beam computed tomography,CBCT)三维成像研究上颌尖牙腭侧埋伏阻生患者邻近侧切牙冠根形态大小特征以探究其病因。方法选择2012年9月至2015年3月四川医科大学附属口腔医院正畸科就诊的错牙合患者84例,对所有患者共计94颗上颌尖牙(实验组:上颌尖牙腭侧埋伏阻生患者24例,27颗阻生尖牙;对照组A:30例上颌尖牙唇侧埋伏阻生患者,37颗阻生尖牙;对照组B:30例上颌尖牙正常萌出患者,30颗尖牙)使用口腔专用CBCT影像扫描。利用CS 3D Imagine软件选择需要的参考平面测量所选尖牙方位及邻近侧切牙形态,对17个测量值[牙体长度、牙冠长度、牙根长度、唇侧釉牙骨质界(cementoenamel junction,CEJ)处近远中径等]进行统计分析。结果实验组牙体长度、牙冠长度、离CEJ4 mm处牙冠近远中径[(21.240±1.651)、(8.150±0.889)、(5.900±0.581)mm]均小于对照组A[(22.930±1.849)、(9.270±0.559)、(7.260±0.579)mm],差异有统计学意义(P0.05);CEJ处近远中径、离CEJ4 mm处牙根颊舌径、CEJ处颊舌径、尖牙牙尖顶到冠状平面、牙合平面的距离亦小于对照组A(P0.05)。实验组牙体长度、牙冠长度、离CEJ4 mm处牙冠近远中径均小于对照组B的[(23.200±0.474)、(9.070±0.534)、(6.680±0.684)mm],差异有统计学意义(P0.05);牙根长度、CEJ处近远中径、CEJ处颊舌径、离CEJ 4 mm处牙根颊舌径、离8 mm处牙根颊舌径、尖牙牙尖顶到矢状平面及牙合平面的距离均较对照组B差异有统计学意义(P0.05)。腭侧阻尖牙牙尖顶到矢状平面、冠状平面、牙合平面的距离、尖牙长轴与矢状平面、冠状平面所成角度与邻近侧切牙的牙根长度、CEJ处颊舌径、离CEJ 4 mm处牙根颊舌径、离8 mm处牙根颊舌径之间存在相关性(P0.05)。腭侧阻生尖牙尖牙顶到矢状平面、牙合平面的距离和年龄存在相关性(r=-0.472、-0.489,P0.05)。结论上颌尖牙腭侧阻生的主要原因之一是邻近侧切牙过小而对尖牙引导不足。年龄越大,上颌尖牙腭侧阻生程度越严重。
[Abstract]:Objective maxillary canine lateral impaction is a common impacted malformation in oral cavity, but its etiology is still controversial. The purpose of this study was to study the morphological and size characteristics of the crowns of adjacent incisors in patients with palatal impacted maxillary canine by conical beam CT (Cone-beam computed tomography,CBCT) in order to investigate its etiology. Methods from September 2012 to March 2015, 84 patients with malocclusion were selected from Department of Orthodontics, affiliated Stomatology Hospital of Sichuan Medical University. A total of 94 maxillary canine teeth (experimental group: 24 maxillary canine side impacted patients, 27 impacted canine teeth, control group A: 30 maxillary canine labial side impacted patients, 37 impacted canine teeth; Control group B: 30 patients with normal maxillary canine eruption and 30 canine teeth) were scanned by oral CBCT. Using CS 3D Imagine software to select the needed reference plane to measure the azimuth of canine teeth and the shape of adjacent lateral incisors, 17 measurements were made [tooth length, crown length, root length].
