口腔扁平苔藓患者唾液中差异蛋白的检测及ABCG2在口腔扁平苔藓癌变过程中的表达研究

【摘要】:第一部分口腔扁平苔藓患者唾液中差异蛋白的检测目的:口腔扁平苔藓(Oral Lichen Planus,OLP)是一种较为常见且发病机制尚不明确的口腔黏膜疾病,该病顽固难治易复发且有一定的恶变倾向。本研究对病理证实为口腔扁平苔藓的患者与健康对照组唾液中的差异蛋白进行研究,以及分析化湿行瘀清热方治疗口腔扁平苔藓不同类型患者前后唾液中差异蛋白的表达情况。探讨口腔扁平苔藓的发病机制及化湿行瘀清热方对口腔扁平苔藓的治疗效果及作用机制。方法:1采用酶联免疫吸附实验(ELISA)方法对口腔扁平苔藓患者(20例)和健康志愿者(20例)唾液中Ig M、sAA、ZAG的含量进行测定,分析IgM、sAA、ZAG在唾液中的表达情况。2采用采用ELISA方法对口腔扁平苔藓糜烂型(10例)和非糜烂型(10例)患者唾液中IgM、sAA、ZAG的含量进行测定,分析Ig M、sAA、ZAG在不同类型OLP患者唾液中的表达情况。3采用ELISA方法对化湿行瘀清热方治疗口腔扁平苔藓患者(20例)前后唾液中IgM、sAA、ZAG的含量进行测定,分析化湿行瘀清热方对OLP相关因子表达的影响及作用机制。4对化湿行瘀清热方治疗OLP不同类型患者(糜烂型与非糜烂型各50例)临床疗效进行观测,并进行统计学分析。结果:1口腔扁平苔藓患者唾液中IgM、sAA、ZAG的表达较对照组健康人唾液中IgM、sAA、ZAG表达水平高,差异有统计学意义(P0.05);2口腔扁平苔藓糜烂型与非糜烂型患者唾液中IgM、sAA的表达水平无明显差异。口腔扁平苔藓糜烂型患者唾液中ZAG的表达较非糜烂型患者唾液中ZAG的表达高,差异有统计学意义(P0.05);3口腔扁平苔藓患者经化湿行瘀清热方治疗后唾液中Ig M、sAA、ZAG的表达水平较治疗前患者唾液中IgM、sAA、ZAG表达水平降低,差异有统计学意义(P0.05);4化湿行瘀清热方治疗口腔扁平苔藓不同类型患者均有良好的临床效果,但糜烂型组与非糜烂型组无统计学差异。第二部分ABCG2在口腔扁平苔藓癌变过程中的表达研究目的:研究ABCG2在口腔扁平苔藓(Oral Lichen Planus,OLP)及口腔扁平苔藓相关鳞状细胞癌(Oral Lichen Planus Related Oral Squamous Cell Carcinoma,OLP-OSCC)组织中的表达情况,并行统计学分析。探讨ABCG2是否可以作为预测OLP癌变的早期检测指标,以期为OSCC患者的临床诊断提供新的标志物。方法:本实验采用免疫组化方法检测30例OLP病变组织、8例OLP-OSCC病变组织中ABCG2蛋白表达情况,并比较ABCG2在OLP组织和OLP-OSCC组织中表达水平的差异。结果:ABCG2在OLP及OLP-OSCC组织中表现为高表达,细胞质黄染,OLP组中的表达水平较OLP-OSCC组低(Z=-4.103,P=0.000)。结论:1 OLP患者唾液中IgM、s AA、ZAG可能参与OLP的发病过程,有望成为OLP的差异蛋白。2 ZAG在OLP糜烂型中较非糜烂型高表达,可能参与了OLP的发展,甚至恶化过程。3化湿行瘀清热方治疗OLP不同类型患者临床效果显著。4 ABCG2高表达与OLP的恶变有一定的相关性,可能积极参与了OLP恶变过程。
[Abstract]:Objective: oral lichen planus (Oral Lichen Planus,OLP) is a common oral mucosal disease with unclear pathogenesis. In this study, the differential proteins in saliva of patients with oral lichen planus proved by pathology and healthy control group were studied, and the expression of differentially expressed proteins in saliva of different types of patients with oral lichen planus before and after treatment with Huashingxingyu Qingre recipe was analyzed. To explore the pathogenesis of oral lichen planus (OLP) and the therapeutic effect and mechanism of Huishingxingyu Qingrefang (HXYQR) on oral lichen planus (OLP). Methods: 1Enzyme-linked immunosorbent assay (ELISA) was used to determine the content of Ig mapsAAZAG in saliva of 20 patients with oral lichen planus (OLP) and 20 healthy volunteers. The expression of IgM,sAA,ZAG in saliva was analyzed. (2) the content of IgM,sAA,ZAG in saliva of patients with oral lichen planus (10 cases) and non-erosive type (10 cases) was determined by ELISA method. To analyze the expression of Ig mecsAAAZAG in saliva of different types of OLP patients. (3) to measure the content of IgM,sAA,ZAG in saliva of 20 patients with oral lichen planus before and after treatment by ELISA method. The effect and mechanism of Huazhingxingyu Qingrefang on the expression of OLP related factors were analyzed. 4 the clinical efficacy of Huazhingxingyu Qingrefang in treating different types of OLP patients (50 cases of erosive type and 50 cases of non-erosive type) was observed and statistically analyzed. Results: 1the expression of IgM,sAA,ZAG in saliva of patients with oral lichen planus was significantly higher than that of healthy controls (P0.05), and the expression of IgM,sAA in saliva of patients with erosive and non-erosive oral lichen planus had no significant difference. The expression of ZAG in saliva of patients with oral lichen planus was higher than that of non-erosive patients. The difference was statistically significant (P0.05); 3 the expression level of Ig mens-AAAZAG in saliva of patients with oral lichen planus was lower than that of patients with oral lichen planus before treatment, and the expression level of IgM,sAA,ZAG in saliva of patients with oral lichen planus was lower than that of patients with oral lichen planus before treatment. The difference was statistically significant (P0.05); 4 the treatment of different types of oral lichen planus patients had good clinical effect, but there was no statistical difference between erosive group and non-erosive group. The second part of the study on the expression of ABCG2 in oral lichen planus carcinogenesis objective: to study the expression of ABCG2 in oral lichen planus (Oral Lichen Planus,OLP) and oral lichen planus associated squamous cell carcinoma (Oral Lichen Planus Related Oral Squamous Cell Carcinoma,OLP-OSCC) tissues and statistical analysis. To explore whether ABCG2 can be used as an early marker for predicting the canceration of OLP, and to provide a new marker for clinical diagnosis of OSCC patients. Methods: immunohistochemical method was used to detect the expression of ABCG2 protein in 30 cases of OLP and 8 cases of OLP-OSCC, and to compare the expression level of ABCG2 in OLP and OLP-OSCC. Results: the expression of ABCG2 in OLP and OLP-OSCC tissues was high and the cytoplasm was yellow stained. The expression level of ABCG2 in OLP group was lower than that in OLP-OSCC group (ZP0. 000). Conclusion: 1 IgM,s AA,ZAG in saliva of OLP patients may be involved in the pathogenesis of OLP and may be the differential protein of OLP. 2 the expression of ZAG in OLP erosive type is higher than that in non-erosive type, and it may be involved in the development of OLP. The clinical effect of Huazhingyu Qingrefang on different types of OLP patients was remarkable. 4 the high expression of ABCG2 was related to the malignancy of OLP, which may be actively involved in the process of OLP malignancy.
