骨性Ⅰ类青少年上气道形态和舌骨位置的锥体束CT分析

【摘要】:目的:分析骨性Ⅰ类均角型(正常骨面型)不同年龄组青少年上气道形态和舌骨位置的差异,以及各年龄组的性别差异;分析骨性Ⅰ类不同垂直骨面型女性青少年上气道形态和舌骨位置的差异。方法:按照纳入标准和排除标准选取2014年7月至2016年3月期间到乌鲁木齐市口腔医院因正畸治疗前检查需要而拍摄CBCT的青少年患者66例,其中骨性Ⅰ类均角型青少年患者40例(男性20例,女性20例),骨性Ⅰ类低角型青少年女性患者13例,骨性Ⅰ类高角型青少年女性患者13例。将患者CBCT图像导入MIMICS16.0软件,完成上气道重建和上气道形态和舌骨位置的测量。40例正常骨面型的青少年患者按年龄分组比较上气道形态和舌骨位置可能存在的年龄和性别差异,并采用两样本t检验进行统计分析。39例不同垂直骨面型女性患者按照GoGn-SN角大小分为3组:低角组(GoGn-SN27.3°);均角组(27.3°GoGn-SN37.7°);高角组(GoGn-SN37.7°),比较不同垂直骨面型女性青少年患者上气道形态和舌骨位置间可能存在的差异,并采用单因素方差分析进行统计学分析。结果:随年龄增长,上气道的各测量项目均有增大,男性上气道L3、LAT3、LAT4、VOL1、VOL4和男性舌骨H-Y、H-EB、H-PNS以及女性上气道L1和L,差异具有统计学意义(P0.05),女性舌骨在各测量项目中差异均无统计学意义(P0.05);在14~16岁组,上气道L4、L、LAT3、VOL4和舌骨H-Y、H-EB、H-PNS,存在性别间差异(P0.05);不同垂直骨面型组女性上气道形态在鼻咽部AP1及腭咽部AP2、R2差异具有显著性(P0.05),上气道长度、横截面、体积等差异无显著性(P0.05);均角组AP2较低角组减小,高角组AP1和AP2均较低角组减小,高角组R2较低角组增大,差异具有显著性(AP1、R2:P0.01;AP2:P0.05)。不同垂直骨面型组女性舌骨位置在C3-Me、H-X、及H-S-Ba差异具有显著性(P0.01),高角组均较低角组减小,差异具有显著性(P0.01)。结论:随年龄增长,男性鼻咽、喉咽体积增长明显,男性舌骨向下移位,14~16岁青少年女性喉咽气道较男性大,但男性舌骨相对于颅底位置较女性更靠下。不同垂直骨面型组女性青少年上气道形态和舌骨位置存在明显差异。
