上颌后牙牙根与毗邻解剖结构关系的锥形束CT分析

【摘要】:目的测量并分析上颌后牙牙根与毗邻解剖结构的位置关系,为临床治疗提供参考依据。方法从山东大学口腔医院CBCT数据库中选取符合纳入标准的患者资料120例,应用GALAXIS软件观察各牙根与上颌窦底的关系分型并统计各类型的发生率,测量根尖至上颌窦底的垂直距离和牙根中点至相邻牙槽骨外板的水平距离并进行统计学分析。结果根尖与上颌窦底有骨板相隔的类型发生率最高;上颌第二磨牙的近中颊根根尖至上颌窦底的垂直距离最短(P0.05);第二前磨牙牙根和第二磨牙腭根根尖至上颌窦底的垂直距离最长(P0.05);上颌第一磨牙的近中颊根距颊侧骨板的水平距离最近(P0.05);上颌第一磨牙的腭根距腭侧骨板的水平距离最近(P0.05)。结论上颌后牙牙根与上颌窦底及牙槽骨板关系密切,在治疗前应仔细观察;CBCT可以清晰显示上颌后牙区各解剖结构间的关系,适用于治疗前的精确诊断和设计。
[Abstract]:Objective to measure and analyze the relationship between the root of maxillary posterior teeth and its adjacent anatomical structure, and to provide reference for clinical treatment. Methods 120 patients were selected from CBCT database of Shandong University Stomatology Hospital. The relationship between each root and maxillary sinus floor was observed by GALAXIS software and the incidence rate of each type was counted. The vertical distance from the root tip to the maxillary sinus floor and the horizontal distance from the midpoint of the root to the adjacent alveolar external plate were measured and statistically analyzed. Results the incidence of the type of bone plate separated from the base of maxillary sinus was the highest, and the vertical distance between the proximal buccal root tip of maxillary second molar and the floor of maxillary sinus was the shortest (P0.05). The vertical distance between the root tip of the second premolar and the palatal root of the second molar and the bottom of the maxillary sinus was the longest (P0.05), and the horizontal distance between the proximal buccal root of the maxillary first molar and the buccal bone plate was the closest (P0.05). The maxillary first molar had the closest horizontal distance from the palatine root to the palatine bone plate (P0.05). Conclusion the root of maxillary posterior teeth is closely related to the floor of maxillary sinus and alveolar bone plate and should be carefully observed before treatment. CBCT can clearly show the relationship between the anatomical structures of maxillary posterior teeth and is suitable for accurate diagnosis and design before treatment.
