口腔种植修复临床效果15年回顾

【摘要】：目的总结评价行骨结合种植体种植修复15年的临床效果。方法回顾性分析行骨结合种植体种植修复2 302例患者临床资料,所有种植体共4 536枚,其中ITI种植体1 395枚,Ankylos种植体1 234枚,Bicon种植体165枚,Nobel种植体612枚,Camlog种植体122枚,BLB种植体765枚,Xive种植体176枚,Frialit-Ⅱ种植体67枚。所有患者的种植修复体上部结构均在2015年12月前完成。观察时间为3~180个月,中位随访时间为48个月。观察方法为影像学检查和临床检查,存留率采用Wheeler存留标准评估,纳入Kaplan-Meier存留曲线统计。结果共2 302例患者,4 536枚种植体,其中随访1 660例,失访642例,失访率为27.89%,失访种植体数为1 405枚,种植体失访率为30.97%。随访中有112例患者共152枚种植体确诊为种植体周围炎。有明确记录的共56枚种植体脱落。15年随访累计修复体松动35件,修复体折断6件,种植体折断1件,烤瓷冠崩瓷62件。据Wheeler存留标准统计种植体15年存留率为96.00%。满意度问卷调查结果显示,种植修复满意率为95.36%。结论研究种植体的15年存留率达到96.00%,种植修复远期效果好,存留价值高,满意率高。导致种植修复失败的主要原因为种植体周围炎。
