Bcl-2、MDM2和P21在口腔黏膜白斑和口腔鳞癌中的表达及相关研究

【摘要】:【目的】 研究细胞癌变相关蛋白B淋巴细胞瘤-2基因(Bcl-2)、泛素蛋白连接酶MDM2、细胞周期依赖性激酶抑制因子P21在人口腔正常黏膜、白斑、鳞状细胞癌细胞中的表达变化,进一步深入研究口腔鳞癌的发生发展机制提供科学依据。 【方法】 采用免疫组化二步法检测正常15例正常口腔黏膜、25例口腔黏膜白斑、25例口腔鳞癌细胞中Bcl-2、MDM2和P21蛋白的表达情况,并通过免疫组化图像定量软件进行表达的相对定量分析,以统计软件对实验结果进行统计学分析。 【结果】 1、Bcl-2、MDM2蛋白在人口腔正常黏膜、单纯增生性白斑、异常增生性白斑和口腔鳞癌细胞中的表达逐渐递增(P<0.05)。 2、P21蛋白在口腔正常黏膜、口腔单纯增生性白斑细胞中呈阳性表达,在口腔异常增生白斑、口腔鳞状细胞癌细胞中呈低水平表达。经统计分析表明口腔异常增生白斑与口腔鳞状细胞癌中P21蛋白表达差异具有统计学意义。 3、利用统计分析表明,在人异常增生性白斑和口腔鳞状细胞癌组织中Bcl-2和MDM2蛋白表达呈正相关,而P21与Bcl-2和MDM2蛋白的表达未见显著相关性。 【结论】 1、Bcl-2、MDM2蛋白在口腔异常白斑和鳞癌中的表达水平较口腔正常黏膜和单纯增生性白斑中的表达明显增高,而P21蛋白在异常鳞癌中的表达水平较口腔正常黏膜和单纯白斑降低,提示Bcl-2、MDM2和P21蛋白表达异常与口腔鳞癌的的发生有关,因此可将Bcl-2、MDM2和P21蛋白作为口腔黏膜病变进展的监测指标,以提高早期口腔癌的发现率。 2、在口腔异常增生性白斑和口腔鳞癌细胞组织中,Bcl-2蛋白和MDM2蛋白的表达呈显著正相关性,提示二者在口腔黏膜癌变中可能有协同作用。 3、虽然Bcl-2、MDM2和P21蛋白三者均是P53通路上P53基因的相关基因,但经统计学分析,,未见P21蛋白的表达与Bcl-2蛋白或MDM2蛋白有相关性,提示P21蛋白对细胞癌变的作用机制不同于二者。
[Abstract]:[objective] to investigate the expression of cell carcinogenesis associated protein B lymphocyte tumor-2 gene (Bcl-2) and ubiquitin ligase MDM2, cell cycle-dependent kinase inhibitor P21 in human normal mucosa and leukoplakia. The changes of expression in squamous cell carcinoma provide scientific basis for further study on the pathogenesis of oral squamous cell carcinoma. [methods] the expression of Bcl-2,MDM2 and P21 protein in 15 cases of normal oral mucosa, 25 cases of oral leukoplakia and 25 cases of oral squamous cell carcinoma were detected by immunohistochemical two-step method. The relative quantitative analysis of the expression was carried out by the image quantitative software of immunohistochemistry, and the experimental results were analyzed statistically with the statistical software. [results] 1. The expression of Bcl-2, MDM2 protein increased gradually in normal mucosa, simple proliferative leukoplakia, dysplasia leukoplakia and oral squamous cell carcinoma (OSCC) (P < 0.05). 2. The expression of P21 protein was positive in normal oral mucosa, simple proliferative leukoplakia cells in oral cavity, and low expression in oral dysplasia leukoplakia and oral squamous cell carcinoma cells. Statistical analysis showed that there was a significant difference in the expression of P21 protein between oral dysplasia leukoplakia and oral squamous cell carcinoma. 3. Statistical analysis showed that there was a positive correlation between the expression of Bcl-2 and MDM2 protein in human dysplasia leukoplakia and oral squamous cell carcinoma, but there was no significant correlation between P21 and Bcl-2 and MDM2 protein expression. [conclusion] 1. The expression level of Bcl-2, MDM2 protein in oral abnormal leukoplakia and squamous cell carcinoma was significantly higher than that in normal oral mucosa and simple proliferative leukoplakia. The expression level of P21 protein in abnormal squamous cell carcinoma was lower than that in normal oral mucosa and simple leukoplakia, suggesting that the abnormal expression of Bcl-2,MDM2 and P21 protein was related to the occurrence of oral squamous cell carcinoma. Therefore, Bcl-2, could be expressed in oral squamous cell carcinoma. MDM2 and P21 proteins were used as indicators to monitor the progression of oral mucosal lesions in order to improve the detection rate of early oral cancer. 2. There was a significant positive correlation between the expression of Bcl-2 protein and MDM2 protein in dysplasia leukoplakia and oral squamous cell carcinoma, suggesting that they might play a synergistic role in the carcinogenesis of oral mucosa. 3. Although both Bcl-2,MDM2 and P21 proteins were related genes of p53 gene in p53 pathway, there was no correlation between P21 protein expression and Bcl-2 protein or MDM2 protein by statistical analysis, but there was no correlation between P21 protein expression and Bcl-2 protein or MDM2 protein. These results suggest that the mechanism of P21 protein on cell carcinogenesis is different from that of P21 protein.
