牙周炎对产妇母婴结局的影响

【摘要】:目的:探讨妊娠期牙周炎对产妇母婴结局的影响及其作用机制。方法:选取行口腔检查的90例孕妇(孕32~38周)为研究对象,根据检查结果分为牙周炎30例,牙龈炎30例以及牙周正常孕妇30例。根据新生儿出生体重分为体重正常组56例和低出生体重组34例,根据分娩孕周分为足月产组62例以及早产儿组28例。采用ELISA法测定孕妇牙龈沟液、唾液及血清中IL-1β,IL-8水平。结果:牙周炎组新生儿出生体重、分娩孕周显著小于牙龈炎组及牙周正常组,而早产儿及低出生体重儿出生率高于牙龈炎组及牙周正常组(P0.05)。牙周炎组牙龈沟液、唾液IL-1β、IL-8水平显著高于牙龈炎及牙周正常组(P0.05)。早产儿组血清中IL-1β、IL-8水平显著高于足月儿组,低出生体重儿血清中IL-1β、IL-8水平显著高于体重出生正常组(P0.05)。经Pearson相关性分析可知,3组龈沟液中IL-1β、IL-8水平与唾液中IL-1β、IL-8水平有统计学相关性(P0.05),而龈沟液中IL-1β、IL-8水平与血液中IL-1β、IL-8水平无相关性(P0.05)。结论:牙周炎孕妇龈沟液、唾液及血清中IL-1β、IL-8水平升高可能与低出生体重儿及早产儿等不良妊娠结局有关。
[Abstract]:Objective: to investigate the effect of periodontitis during pregnancy on maternal and infant outcome and its mechanism. Methods: 90 pregnant women (32 weeks gestation for 38 weeks) were selected as subjects. According to the results, 30 cases of periodontitis, 30 cases of gum inflammation and 30 cases of normal periodontitis were divided into three groups: periodontitis (30 cases), gum inflammation (30 cases) and normal periodontal pregnant women (30 cases). According to the birth weight of neonates, they were divided into normal weight group (n = 56) and low birth weight group (n = 34), full-term delivery group (n = 62) and premature infants group (n = 28). The levels of IL-1 尾 and IL-8 in gum sulcus fluid, saliva and serum of pregnant women were measured by ELISA. Results: the birth weight and gestational age of neonates in periodontitis group were significantly lower than those in gingivitis group and normal periodontal group, while the birth rate of premature infants and low birth weight infants was higher than that of gingivitis group and normal periodontal group (P 0.05). The levels of IL-1 尾 and IL-8 in gum crevicular fluid and saliva in periodontitis group were significantly higher than those in gingivitis group and normal periodontal group (P 0.05). The levels of serum IL-1 尾 and IL-8 in premature infants were significantly higher than those in term infants, and the levels of IL-1 尾 and IL-8 in low birth weight infants were significantly higher than those in normal birth weight group (P 0.05). The results of Pearson correlation analysis showed that the levels of IL-1 尾 and IL-8 in gingival crevicular fluid were significantly correlated with the levels of IL-1 尾 and IL-8 in saliva (P 0.05), but the levels of IL-1 尾 and IL-8 in gingival crevicular fluid were not correlated with the levels of IL-1 尾 and IL-8 in blood (P 0.05). Conclusion: the increased levels of IL-1 尾 and IL-8 in gum crevicular fluid, saliva and serum of pregnant women with periodontitis may be related to the adverse pregnancy outcomes of low birth weight infants and premature infants.
