太原市居民伤害死亡状况与损失分析

【摘要】: 目的了解太原市居民伤害死亡的水平、构成及潜在损失相关情况。 方法对太原市2003年伤害死亡监测资料,用描述性流行病学方法进行伤害死亡情况和死因分析,即计算死亡率、标化死亡率、死亡构成及顺位。用卫生经济学方法研究伤害死亡引起的潜在损失,即计算各种死因“早死”导致的潜在寿命损失年数、潜在工作损失年数、潜在经济损失值和校正潜在价值损失年数,以及占总死亡的比例;平均死亡年龄、平均潜在寿命损失年数、平均潜在工作损失年数、平均潜在经济损失值和平均校正潜在价值损失年数。死因分类采用中国疾病分类(CCD)损伤和中毒外部原因分类。标化死亡率根据2000年全国人口普查标准人口年龄构成对数据进行标化。所有数据经核对后输入计算机建立Excel数据库,用SPSS 11.5软件进行统计分析。 结果太原市居民伤害标化死亡率为36.14/10万,男性为52.77/10万,女性为17.85/10万,差异有统计学意义,死亡率随年龄的增加,呈上升趋势。死亡人数在总死亡中的构成比为7.86%,在30-39岁年龄组达到高峰,季节分布不明显,顺位居全死因第4位;交通事故、工伤事故、中毒、外伤、加害是伤害的主要死因。 因伤害“早死”导致的潜在寿命损失年数为13254人年、潜在工作损失年数为9567人年、潜在经济损失值为15185.7万元、校正潜在价值损失年数为11940人年,分别是总死亡导致损失的24.26%、32.12%、32.12%和1.75倍。伤害死亡人群的平均死亡年龄为41.09岁,平均潜在寿命损失年数为35.73年、平均潜在工作损失年数为28.14年、平均潜在经济损失值为44.67万元、平均校正潜在价值损失年数为31.84年。 在各种损失的比较中,男性居民的损失程度都大于女性。不论男女,交通事故的损失程度都是最严重的,居于首位;第二位的损失,男性为工伤事故,女性为中毒。 结论伤害是可以通过一定的措施来预防和控制的。目前,伤害是严重危害太原市居民身体健康和生命安全的重要问题之一,由于“早死”,使伤害死亡带来更多的劳动力和其它潜在损失,尤其是交通事故、工伤事故和中毒,应该引起重视。应针对太原市的伤害死亡谱和目标人群制订有效的控制策略和措施。伤害的成功控制,最终需要政府行为的引导和干预。
[Abstract]:Objective to investigate the level, composition and potential loss of injury death in Taiyuan. Methods the data of injury death surveillance in Taiyuan in 2003 were analyzed by descriptive epidemiological method, namely, the mortality rate, standardized mortality rate, death composition and rank were calculated. Using the method of health economics to study the potential loss caused by injury death, that is, to calculate the number of years of potential life lost, the number of years of potential work loss, the value of potential economic loss and the number of years of corrected potential value lost due to "premature death" of various causes of death. The average age of death, the average number of years of potential life lost, the average number of years of potential work loss, the average value of potential economic loss and the average number of years of corrected potential value of loss. The causes of death were classified by Chinese disease classification (CCD) injury and external causes of poisoning. The standardized mortality rate was standardized according to the standard age composition of the population in the 2000 national census. All the data were checked and input into the computer to establish Excel database, SPSS 11.5 software for statistical analysis. Results the standardized mortality rate of injury was 36.14 / 100 000 in Taiyuan, 52.77% / 100 000 in male and 17.85 / 100 000 in female. The mortality rate increased with the increase of age. The proportion of the number of deaths in the total death was 7.86%, which reached the peak in the age group of 30-39 years old, and the seasonal distribution was not obvious, which was the fourth leading cause of death. Traffic accident, industrial injury, poisoning, injury and injury were the main causes of death. The number of years of potential life lost, the number of years of potential work loss, the value of potential economic loss and the corrected number of years of potential value lost are 13254 person-years, 9567 person-years, 151.857 million yuan and 11940 person-years, respectively. 24.26%, 32.12% and 1.75 times of the total loss caused by death, respectively. The average age of death was 41.09 years old, the average years of potential life lost was 35.73 years, the average number of years of potential work loss was 28.14 years, the average value of potential economic loss was 446700 yuan, the average number of corrected potential value years of loss was 31.84 years. In comparison of various losses, the extent of loss of male residents was greater than that of women. No matter male or female, the degree of loss of traffic accident is the most serious, occupying the first place; the second place of loss, male is industrial injury accident, female is poisoning. Conclusion injury can be prevented and controlled by certain measures. At present, injury is one of the important problems that seriously endanger the health and safety of Taiyuan residents. As a result of "early death", injuries and deaths bring more labor force and other potential losses, especially traffic accidents, industrial accidents and poisoning. Attention should be paid to it. The effective control strategies and measures should be made for the death spectrum of injury and the target population in Taiyuan. The successful control of injury ultimately needs the guidance and intervention of government behavior.
