不同分期胃癌患者组织细胞氧化应激损伤的比较

【摘要】:目的 氧化应激损伤在肿瘤的发生及发展中起着促进作用已被学术界认可,关于胃癌的研究也发现胃癌患者肿瘤组织及血浆氧化应激产物浓度较对照组存在明显差异。本研究通过检测分析不同临床分期胃癌患者肿瘤组织及非瘤胃组织氧化应激相关指标的差异,揭示氧化应激损伤与胃癌进展的关系,,为未来胃癌检测分期及肿瘤抗氧化治疗提供理论及临床依据。 方法 选择2012年7月-2014年3月我院的收治并行胃癌根治术的胃癌患者60例。病例均为术前胃镜活检确诊为胃癌,影像学未发现远处转移表现,无术前辅助化疗史和抗癌药物等抗氧化药物治疗史,手术指征明确且排除手术禁忌、术后病理证实为I-III期的胃腺癌患者。并根据其术后病理分期,分别分成I期组、II期组、III期组。取术后标本肿瘤中心及非瘤胃组织,检测其丙二醛(malonaldehyde,MDA)浓度和超氧化物歧化酶(superoxide dismutase,SOD)活性,分析并比较数据间差异。 结果: 1.本次研究共入组病例60例。其中I期组12例(男性8例,女性4例);年龄(58.1土10.7)岁;手术方式腹腔镜5例,开放7例;手术切除范围10例行胃全切,2例行远端胃切除。II期组共20例(男13例,女性7例);年龄(63.1土8.3)岁;手术方式腹腔镜9例,开放11例;手术切除范围16例行胃全切,4例行远端胃切除。III期组28例(男性19例,女性9例);年龄(64.8土8.8)岁;手术方式腹腔镜13例,开放15例;手术切除范围全胃23例,5例行远端胃切除。三组病例间除I期组年龄分布与II、III期组间有显著差异(P0.05)外,组间年龄分布、男女性别比例、手术方式、手术切除术范围均无显著性差异。 2. I期组麻醉时间(麻醉诱导开始至麻醉拔管时间)(253.2土48.3min),肿瘤血供阻断时间(离断胃网膜右血管至肿瘤整体切除时间)(68.1土15.4min),术中出血量(98土36.4ml);II期组麻醉时间(267.2土53.1min),肿瘤血供阻断时间(72.3土25.2min),术中出血量(103.5土42.1ml);III期组麻醉时间(271.2土44.2min),肿瘤血供阻断时间(69.4土29.1min),术中出血量(123.1土58.1ml).三组间术中资料对比,麻醉时间、肿瘤血管阻断时间、术中出血量均无显著性差异(P0.05)。 3.三组各组内对比:MDA浓度三组间肿瘤中心组织与非瘤胃组织对比,前者浓度更高,组内对比差异均有统计学意义(P0.05);SOD活性肿瘤中心组织与非瘤胃组织对比,前者水平较低,组内对比差异有统计学意义(P0.05)。 4.三组间对比:肿瘤组织及非瘤胃组织中MDA浓度对比,I期组较II、III期组浓度明显偏低,差异有统计学意义(P0.05);II期组较III期组浓度偏低,差异有统计学意义(P0.05);三组间MDA水平呈逐渐升高趋势。肿瘤组织SOD活性对比,I期组较II、III期组水平明显偏低,差异有统计学意义(P0.05);II期组与III期组间差异不明显(P0.05)。非瘤胃组织SOD活性对比,I期组较II、III期组水平偏低,差异有统计学意义(P0.05);II期组较III期组间水平偏低亦(P0.05);三组间SOD浓度呈升高趋势。 结论 1、不同时期胃癌患者其肿瘤组织氧化应激损伤较癌旁非瘤胃组织均有加重表现。 2、随着肿瘤进展,肿瘤组织及瘤旁非瘤组织其氧化应激损伤总体呈逐渐加重趋势。 3、肿瘤组织其抗氧化损伤活性逐渐加强,但其最高活性存在限制。
[Abstract]:objective
Oxidative stress in the occurrence and development of tumor plays an important role in the academic circles has been recognized, studies on gastric cancer also found that oxidative stress concentration of gastric cancer tissue and plasma compared with the control group, there are obvious differences. In this study, through the analysis of the difference of corresponding indexes of oxidative stress in different clinical stages of gastric cancer tissues and non rumen tissue the relationship between oxidative stress and the progression of gastric cancer, and provide theoretical and clinical basis for future detection of gastric cancer staging and tumor antioxidant treatment.
Method
In July 2012 -2014 year in March in our hospital were parallel 60 gastric cancer patients with radical resection of gastric cancer cases. The cases were diagnosed as gastric cancer by preoperative biopsy and imaging without distant metastasis, no preoperative adjuvant chemotherapy drug treatment history and history of anti cancer drugs, surgical indications and contraindication clear exclusion, postoperative pathologic stage I-III gastric cancer patients. According to the postoperative pathological staging, were divided into I group, II group, III group and non tumor specimens. The center of rumen tissue after operation, the detection of malondialdehyde (malonaldehyde, MDA) concentration and superoxide dismutase (superoxide dismutase, SOD) the difference between activity data and analysis.
