心脏型脂肪酸结合蛋白在早期大鼠心肌损伤坏死时的含量的动态变化

【摘要】:目的:构建大鼠急性心肌损伤坏死模型的基础上,检测不同时间点血清中心脏型脂肪酸结合蛋白(H-FABP)的浓度变化,通过与肌钙蛋白I(cTnI)对比,探讨H-FABP对早期诊断急性心肌梗死的价值。 方法:健康雄性Sprauge Dawley(SD)大鼠(体重220±20克)多点皮下注射异丙肾上腺素10mg/kg,每24小时注射一次,连续三天,制作大鼠心肌损伤坏死模型。对照组给予相同量生理盐水(10mg/kg)。选取SD大鼠30只,,随机分成模型组和对照组,20只进入模型组,10只进入对照组。主要操作步骤如下:术前大鼠禁食禁水4小时,称重后,以1.5%戊巴比妥钠(3ml/kg)腹腔注射麻醉大鼠后,待肌力减弱时将大鼠固定在手术台上。打开RM6240多通道生理信号采集系统,设定好参数,将大头针形插入大鼠四肢皮下,左上肢连接红色正极,右上肢接绿色负极,参考极黑色接右下肢,记录Ⅱ导联心电图。行气管插管和颈动脉分离后,观察心电图基线,待心电图稳定后开始描记心电图。接着再次予末次多点皮下注射异丙肾上腺素(10mg/kg),同时观察心电图,记录不同时间点心电图,观察J点改变为指标。并于末次给药后1小时、3小时、6小时、8小时经颈总动脉插管采血,每次约0.5ml,离心后取上清液分装及做好标记后立即送入-80℃冰箱冻存。对照组给予相同量生理盐水(10mg/kg),余手术操作同模型组。均采用酶联免疫吸附试验法(ELASA法)分别测定H-FABP、cTnI浓度,比较H-FABP、cTnI在两组中的变化。 结果:1.动物存活情况:造模过程中死亡4只,死亡率为13.33%,其中1只死于出血,1只死于气道阻塞,窒息而死亡,1只死于室颤,1只死于麻醉过量。2.病理学检查:模型组心肌病理切片上可见心肌嗜伊红性较对照组增强,心肌纤维肿胀变性,肌纤维断裂,血管扩张充血,心肌细胞可见核碎裂、核溶解,甚至出现点状、片状坏死,间质中可见大量炎性细胞浸润。3.心电图检查:行心肌损伤模型均可出现心电图缺血梗死图形变化,如:J点改变、T波改变,部分Q波形成。4.各组大鼠血清cTnI含量的比较:组内不同时间点比较:对照组各时间点差异无统计学意义(P>0.05);模型组1h与3h组两者无统计学意义(P>0.05),6h、8h与1h之间差异有统计学意义(P<0.01);组间不同时间点比较:除1h、3h外,模型组与对照组比较差异有统计学意义(P<0.01)。5.各组大鼠血清H-FABP含量的比较:组内不同时间比较:对照组1h与3h两组之间差异无统计学意义(P>0.05),6h、8h与1h之间差异有统计学意义(P<0.01);实验组各时间点之间差异有统计学意义(P<0.01);组间不同时间点比较:除1h、3h外,模型组与对照组之间有统计学意义(P<0.01); 结论:1.在异丙肾上腺素诱导心肌坏死模型中,肌钙蛋白在末次给药后3小时以内尚未升高,H-FABP在3小时内升高,早于肌钙蛋白。2. H-FABP可作为异丙肾上腺素诱导大鼠心肌坏死早期诊断的指标。
[Abstract]:Aim: to study the changes of cardiac fatty acid binding protein (H-FABP) in serum at different time points on the basis of rat model of acute myocardial injury and necrosis, and to compare it with cardiac troponin I ~ (cTnI). To investigate the value of H-FABP in early diagnosis of acute myocardial infarction. Methods: healthy male Sprauge Dawley SD rats (weight 220 卤20g) were injected with isoproterenol 10mg / kg subcutaneously every 24 hours for three consecutive days. The rat model of myocardial injury and necrosis was made. The control group was given the same amount of normal saline 10 mg / kg. 30 SD rats were selected. The rats were randomly divided into model group (n = 20) and control group (n = 10). The main steps were as follows: the rats were fasting for 4 hours before operation. After weighing, 1.5% pentobarbital sodium was injected intraperitoneally into anesthetized rats. When the muscle strength was weakened, the rats were fixed on the operating table.
