114例老年患者行关节置换的资料分析与不良事件探讨

【摘要】：目的探讨老年患者行关节置换手术的围术期管理与可能引起术后不良事件的危险因素,为老年患者关节置换手术提供安全保障。方法回顾性分析昆明医科大学第一附属医院骨科2012年2月—2014年4月收治并参与卫生部行业基金《围术期老年人病情评估与风险对策》研究的114例老年髋膝关节置换患者的病例,通过对老年患者围手术期监护及治疗手段的评估干预,获得详尽的资料数据,从术前病例选择、围术期处理、术后功能锻炼相关情况、患者出院前一天临床转归的评估等方面综合分析。结果年龄是术后不良情况的危险因素,年龄≥70岁的患者术后发生不良情况的可能性较70岁的患者高；术前是否有伴随疾病,麻醉方式以及手术方式对术后是否发生并发症无显著影响。结论通过认真的术前评估,按照老年人围术期处理操作流程对老年患者术前合并疾病进行积极处理,选择安全、有效、个体化的麻醉方式,加强术中麻醉管理,有效处理术中不良事件,积极预防术后不良反应,可以确保其围术期的安全。
[Abstract]:Objective to investigate the perioperative management and risk factors of postoperative adverse events in elderly patients undergoing joint replacement surgery. Methods retrospective analysis was made on the clinical data of the orthopedic department of the first affiliated hospital of Kunming medical university from February 2012 to April 2014 and to participate in the evaluation of the condition of the elderly during the perioperative period under the trade fund of the ministry of health. A study of 114 elderly patients with hip and knee arthroplasty. Through the evaluation and intervention of perioperative monitoring and treatment for elderly patients, detailed data were obtained, and the relevant information of preoperative case selection, perioperative treatment and postoperative functional exercise were obtained.
