超声弹性成像评价肝纤维化病理分期的实验研究

【摘要】:目的:本研究通过探讨超声弹性成像参数与肝纤维化病理分期的关系,评估超声弹性成像参数诊断肝纤维化程度的可行性及其病理分子生物学基础。 资料与方法:实验分组:54只新西兰兔皮下注射硫代乙酰胺(thioacetamide,TAA)诱导肝纤维化作为模型组[1~2],经病理组织检查后分为S0组、S1组、S2组、S3组和S4组,另取8只新西兰兔为正常对照组(normal,N)。实验兔麻醉后,肝区去毛,立位检查,选择剑突下肝左叶,避开肝内大血管,距肝被膜0.5~1.0cm,深度4cm,二维取好切面,转换至弹性成像,感兴趣区(regionof interest, ROI)周长46.1mm,面积1.3cm2,出现5个连续稳定波谷,选取其中一个波谷形成的弹性图,得到11个弹性特征量,重复操作3次,取平均值,通过软件计算得出肝纤维化指数(liver fibrosis index,LF index)。处死实验兔,取其肝左叶组织,行HE染色、网染及Masson染色,根据病理检查结果进行分期(S0~S4期)。另取肝组织于㧟80℃液氮冻存,行western-blot检测I型胶原(collagen I,Col I)、Ⅲ型胶原(collagen Ⅲ,Col Ⅲ)表达水平。分析LF index与肝纤维化病理分期的相关性以及肝组织ColⅠ、Col Ⅲ表达水平与肝纤维化病理分期的相关性。 结果: 1.肝纤维化病理分期:S0期7只,S1期14只,S2期6只,S3期7只,S4期5只。 2.超声弹性特征参数随肝纤维化病理分期加重呈增高趋势,,其中LF index与肝纤维化病理分期的相关性最好(r=0.718、P<0.05)。 3.N~S2组与S3~S4组相比ROC曲线下面积为0.983,LF index值为3.11时,敏感性和特异性最大,分别达到83.3%、97.1%。 4.肝组织ColⅠ、Col Ⅲ表达水平随肝纤维化病理分期加重总体呈增高趋势,肝组织ColⅠ、Col Ⅲ表达水平与肝纤维化病理分期呈正相关(r=0.693,P<0.05;r=0.611,P<0.05)。 结论:LF index随肝纤维化病理分期呈显著正相关,超声弹性成像有望成为无创评估肝纤维化程度的新技术。
[Abstract]:Objective: to investigate the relationship between ultrasonic elastic imaging parameters and pathological stages of liver fibrosis and to evaluate the feasibility and pathological molecular biological basis of ultrasonic elastic imaging parameters in diagnosing the degree of hepatic fibrosis. Materials and methods: liver fibrosis was induced by subcutaneous injection of thioacetamide-thioacetamide (TAA) into 54 New Zealand rabbits. After pathological examination, liver fibrosis was divided into S0 group, S1 group, S2 group, S3 group and S4 group, and 8 New Zealand rabbits were taken as normal control group. After anaesthesia, the liver area was removed, the liver was examined by standing position, the hepatic Zuo Ye was selected under the xiphoid process, the large hepatic vessels were avoided, the depth was 4 cm, the two dimensional section was taken and converted to elastic imaging. The region of interest (ROI) is 46.1mm in circumference and 1.3cm ~ 2 in area. There are five continuous stable troughs. The liver fibrosis index (fibrosis) was calculated by software. The experimental rabbits were killed and the liver Zuo Ye tissues were taken for HE staining, net staining and Masson staining. According to the pathological results, stage S0 S4 was performed.
