BIS指导下七氟醚吸入麻醉复合臂丛神经阻滞在小儿上肢手术的应用

【摘要】:目的探讨脑电双频指数(BIS)监测对七氟醚吸入麻醉复合臂丛神经阻滞在小儿上肢手术的指导意义。方法将择期行上肢手术小儿50例,随机分为BIS反馈调控七氟醚组(Ⅰ组)和根据经验用药组(Ⅱ组),每组25例,Ⅰ组在BIS监测下指导术中七氟醚吸入麻醉维持BIS在(65±5),Ⅱ组吸入七氟醚诱导小儿睫毛反射消失后依临床经验来调节七氟醚吸入浓度。两组小儿均于麻醉诱导完成后行超声引导下肌间沟臂丛神经阻滞。观察并记录各时间点MAP、HR、Sa O2,记录每5min BIS值,观察两组小儿麻醉诱导时间、苏醒时间、恢复室停留时间及七氟醚用量,追踪观察术后小儿家属满意程度。结果两组小儿麻醉开始时T1、出恢复室时T6,MAP、HR、Sa O2、BIS,组间比较差异无统计学意义(P0.05),诱导完成时T2、神经阻滞时T3、麻醉结束时T4、苏醒时T5,Ⅰ组BIS值比Ⅱ组高,Ⅱ组MAP、HR变化大于Ⅰ组,差异有统计学意义(P0.05),麻醉诱导时间T1-T2、苏醒时间T4-T5、恢复室停留时间T5-T6及七氟醚用量Ⅰ组少于Ⅱ组,差异有统计学意义(P0.05)。结论 BIS指导下七氟醚吸入麻醉复合臂丛神经阻滞,可降低七氟醚吸入量,有助于判断全麻深度,有利于小儿术后的恢复。
