重组人生长激素对梗阻性黄疸大鼠肠黏膜紧密连接蛋白claudin-3及PKA的影响

【摘要】:目的:对梗阻性黄疸早期应用重组人生长激素(recombinant human growthhormone, rhGH)的实验研究基础上,采用梗阻性黄疸大鼠模型,模拟临床上梗阻性黄疸患者肠黏膜上皮损伤的病理生理机制,研究紧密连接蛋白(tight junction associated protin, TJP) claudin-3和蛋白激酶-A (protein kinase A,PKA)在梗阻性黄疸大鼠肠黏膜的表达,探讨其作用机制。 方法:将63只健康wistar大鼠,雌雄不分,随机分为5组:Ⅰ=假手术组(sham operated,简称S0组)共计15只:Ⅱ=胆总管结扎(common bile duct ligation,简称CBDL)7天组共计12只;Ⅲ=CBDL14天组共计11只;IV=CBDL+rhGH(简称rhGH组)7天组共计13只;V=rhGH14天组共计12只。CBDL组与rhGH组麻醉后游离出胆总管后用4.0丝线结扎,使胆总管被阻断,制成梗阻性黄疸模型组。rhGH组从术后第一天开始每日予腹腔内注射rhGH(上海复蒙基因生物科技有限公司提供),S0组及CBDL组以等量的生理盐水作对照。实验7天及14天后分别检测各组肝功能指标的变化及应用免疫组化、光镜下半定量分析等方法观察末端回肠小肠黏膜形态学改变,并观测紧密连接蛋白claudin-3及PKA在末端回场黏膜上皮细胞上的表达情况。应用SPSS17.0统计软件进行数据的统计分析。 结果: 1) CBDL组、rhGH组较SO组相比,TBIL、DBIL、ALT水平有显著增高(P0.05):CBDL组与rhGH组相比TBIL、DBIL、ALT水平显著增高,有差异性(P0.05);CBDL14天组较CBDL7天组各项肝功能水平增高,有统计学意义(P0.05); rhGH组随时间的延长无统计学意义(P0.05)。 2)CBDL组、rhGH组较s0组相比,肠黏膜严重损伤,有显著的差异性(P0.05);CBDL组随梗阻时间推移肠黏膜损伤程度进行性加重(P0.05);rhGH组较CBDL组各时段肠黏膜损伤程度明显降低,有显著的统计学意义(P0.05);rhGH组随时间的推移无统计学意义(P0.05)。 3) CBDL组、rhGH组较S0组相比肠黏膜紧密连接蛋白claudin-3的表达水平显著降低,有明显的统计学意义(P0.05);CBDL组较rhGH组各时段claudin-3的表达水平显著降低,有差异性(P0.05);rhGH组中随时间的延迟,claudin-3的表达水平略有增高,但无显著差异(P0.05);CBDL组中随着时间的推移claudin-3的表达水平降低,有统计学意义(P0.05) 4)CBDL组、rhGH组较S0组相比肠黏膜紧密连接蛋白PKA的表达水平显著增高,有明显的统计学意义(P0.05):CBDL组较rhGH组各时段PKA的表达水平显著增高,有差异性(P0.05):rhGH组中随时间的延迟,PKA的表达水平略有减低,但无显著差异(P0.05):CBDL组中随着时间的推移PKA的表达水平增高,有统计学意义(P0.05)。 结论: 1.rhGH能上调梗阻性黄疸大鼠肠黏膜紧密连接蛋白claudin-3表达。 2.梗阻性黄疸大鼠可能通过PKA信号传导通路损伤肠黏膜屏障功能。
