基层医院癌痛麻醉药品使用及其认知度的调查分析

【摘要】：目的评价余姚市各基层医院癌痛麻醉药品应用及医务人员对癌痛麻醉药品的认知度情况。方法对余姚市16家基层医院2013年癌痛麻醉药品品种配备情况、用药频度(DDDs)和药物利用指数(DUI)统计分析;并每月随机调取癌痛麻醉药品处方20张,对不合理处方比例及存在问题进行分析;同时随机抽取各家医院医务人员各6名作癌痛麻醉药品认知度调查。结果有8家基层医院癌痛麻醉药品应用以吗啡缓释片为主,但除F院DUI值=1外,其余均小于1;H、P院芬太尼透皮贴DUI值明显大于1,分别为7.20和8.05。不合理处方张数为522张,占55.7%,哌替啶用于癌痛是处方不合理的主要因素,占43.1%;医务人员对阿片类耐受概念、癌痛麻醉药品滴定及如何转换知晓程度较低。结论基层医院癌痛麻醉药品管理及医务人员认知度有待进一步提高。
[Abstract]:Objective to evaluate the use of cancer pain narcotic drugs in primary hospitals of Yuyao City and the recognition of cancer pain drugs among medical staff. Methods the types of narcotic drugs for cancer pain in 16 primary hospitals in Yuyao City in 2013 were analyzed. The frequency of drug use (DDDs) and the drug utilization index (DUI) were statistically analyzed. At the same time, 20 prescriptions of cancer pain drugs were randomly selected every month to analyze the unreasonable prescription proportion and the existing problems, and at the same time, 6 medical staff in each hospital were randomly selected to investigate the cognition of cancer pain narcotic drugs. Results morphine sustained-release tablets were mainly used in 8 primary hospitals, except Dui = 1 in F hospital, the DUI values of fentanyl transdermal adhesive in P hospital were significantly higher than 1, which were 7.20 and 8.05, respectively. The number of unreasonable prescriptions was 522, accounting for 55.7%, and the use of pethidine in cancer pain was the main factor of unreasonable prescription, accounting for 43.1%, the concept of opioid tolerance, titration of cancer pain narcotic drugs and how to change knowledge were relatively low among medical staff. Conclusion the management of cancer pain narcotic drugs and the awareness of medical staff in primary hospitals need to be further improved.
