新型一次性牙垫对麻醉及急危重患者抢救的价值

【摘要】：目的探讨新型改良式一次性牙垫对麻醉及急危重患者抢救的价值。方法将2010年8月~2012年2月行气管插管全身麻醉和急危重抢救行紧急气管内插管的308例患者随机分为观察组156例和对照组152例,对照组采用常规一次性牙垫固定气管导管,观察组采用新型改良式一次性牙垫固定气管导管。分别于插管固定前和固定2 h、4 h、6 h、8 h时,评价两组牙菌斑指数(PLI)、牙龈指数(GI),并比较呼吸机相关性肺炎发生率及患者舒适度。结果气管导管固定后,两组PLI和GI均显著增长(P0.05),但观察组各时点PLI和GI均显著低于同时点对照组(P0.05)。观察组固定气管导管操作时间显著少于对照组,导管移位、牙垫松脱、口腔黏膜损伤、口腔感染、呼吸机相关性肺炎发生率均显著低于对照组(P0.05),患者舒适度显著优于对照组(P0.05)。结论新型改良式一次性牙垫固定气管插管导管,其操作简单、不易脱落、口腔并发症少,能够显著降低呼吸机相关性肺炎的发生率,值得临床推广。
[Abstract]:Objective to explore the value of a new modified one-time dental pad in anesthesia and emergency rescue. Methods from August 2010 to February 2012, 308 patients undergoing endotracheal intubation under general anesthesia and emergency rescue were randomly divided into observation group (156 cases) and control group (152 cases). In the observation group, a new modified one-time dental pad was used to fix the trachea catheter. The plaque index (PLI),) gingival index (GI),) was evaluated before and at 2 h, 4 h, 6 h, 8 h after intubation. The incidence and comfort of ventilator-associated pneumonia were compared between the two groups. Results after tracheal catheter fixation, PLI and GI increased significantly in both groups (P0.05), but PLI and GI in observation group were significantly lower than those in control group at each time point (P0.05). The operation time of fixed trachea catheter in the observation group was significantly shorter than that in the control group, catheter displacement, loosening of the dental pad, oral mucosal injury, oral infection and ventilator-associated pneumonia were significantly lower than those in the control group (P0.05). The comfort of the patients was significantly better than that of the control group (P0.05). Conclusion the new modified one-time dental pad fixed tracheal intubation catheter is simple in operation, not easy to fall off, less complications in oral cavity, and can significantly reduce the incidence of ventilator-associated pneumonia, which is worthy of clinical popularization.
