单侧上尿路结石患者双侧肾盂尿纳米细菌的检测及意义

【摘要】:目的:探讨纳米细菌(nanobacteria,NB)在上尿路结石患者肾脏中的感染情况及两者的相关性。方法:选取单侧肾结石患者27例,分别采集患侧及健侧肾盂尿液,正常对照者27例采集其中段晨尿,分别进行NB的培养,应用茜素红染色、扫描电镜、免疫荧光等方法对培养的NB予以观察和鉴定,比较结石患者和正常对照者纳米细菌的感染情况,以及单侧结石患者健侧和患侧肾脏纳米细菌感染情况,同时分析结石成分,探讨其与纳米细菌的关系。结果:27名单侧上尿路结石患者患侧肾盂尿液样本(27例)中有22例(81.5%)NB培养阳性,健侧肾盂尿液样本(22例)有9(40.9%)例NB培养阳性,两者间差距有显著统计学意义(χ2=8.586,P=0.003)。正常对照组27例中段尿样本中6例(22.2%)培养NB阳性,与结石组患侧肾盂尿样本NB阳性率比较差异有统计学意义(χ2=18.989,P0.001)。18例上尿路结石中,结石成分以草酸钙结石与碳酸磷灰石混合性结石为主约占55.6%,其次为纯草酸钙结石,约为38.9%。结论:纳米细菌感染与上尿路结石发生具有一定相关性,并且纳米细菌在泌尿系结石的发展中可能具有病因学作用。
[Abstract]:Objective: to investigate the relationship between the infection of nanobacteriae and NB in the kidney of patients with upper urinary tract calculi. Methods: 27 patients with unilateral renal calculi were selected. Urine samples were collected from the affected and healthy renal pelvis, and 27 cases of normal control were collected. NB culture was carried out respectively. Alizarin red staining was used and scanning electron microscope (SEM) was used. The cultured NB was observed and identified by immunofluorescence. The infection of nanobacteria in patients with calculi and normal controls and in healthy and affected kidneys of patients with unilateral calculi were compared. Results among the 27 cases of urolithiasis on the upper side of the list, 22 cases were positive for NB culture. In 22 cases of renal pelvis and urine, 9 cases were positive for NB culture, and the difference between them was statistically significant (蠂 2 = 8.586). In normal control group, 6 of 27 cases of urine samples were positive for NB. There was significant difference in NB positive rate between the patients with calculi and the patients with calculi (蠂 2 + 18.989) (P 0.001). 18 cases with upper urinary calculi. Calcium oxalate stone and carbonate apatite mixed stone accounted for 55.6%, followed by pure calcium oxalate stone. Conclusion: there is a certain correlation between the infection of nanometer bacteria and the occurrence of upper urinary tract stones, and the nanometer bacteria may play an etiological role in the development of urinary calculi.
