单纯选择性绿激光汽化术治疗腺性膀胱炎疗效分析（附31例报道）

【摘要】：目的：探讨单纯选择性绿激光汽化术治疗腺性膀胱炎的有效性与安全性。方法：本项研究资料为山东省立医院泌尿微创中心自2010年10月至2013年8月收治的31例腺性膀胱炎病人,其中男性12例,女性19例,年龄26-63岁,平均45.8岁；临床表现为尿频、尿急、尿痛等尿路刺激症状者20例,其中严重影响正常生活工作者3例,排尿困难者6例,伴下腹部疼痛者3例,镜下及肉眼血尿者9例；B超结果示5例膀胱粘膜结节样隆起,14例局部粘膜增厚,4例呈实性突起；31例均经过膀胱镜检查及病理活检,病理结果均为腺性膀胱炎,31例均采用选择性绿激光汽化术治疗,所有病例均不行膀胱灌注治疗,术后第1年每3个月复查膀胱镜,1年以后每6个月复查膀胱镜,全组随访6-30个月,平均16个月,膀胱镜检查如有异常即取活检,通过结果以证实是否复发。本项研究回顾性分析所有病例的围手术期资料、术后并发症、术后膀胱镜检查及病理活检结果,验证该术式的安全性与有效性。结果：31例手术均顺利完成,平均手术时间32.1min,31例手术术中均无明显出血,术后患者尿急、尿痛、尿频及下腹部疼痛等症状均消失,术后均未出现手术并发症,术后平均住院天数6.7天。经过6-30个月随访,其中27例治愈,3例好转,1例复发,无1例癌变。结论：单纯绿激光汽化术治疗腺性膀胱炎具有治愈率高、手术时间短、术后恢复快等优势,是一种有效的方法；单纯绿激光汽化术治疗腺性膀胱炎安全,术后并发症少。
[Abstract]:Objective: to investigate the efficacy and safety of selective green laser vaporization in the treatment of cystitis glandularis. Methods: from October 2010 to August 2013, 31 patients with cystitis glandularis were enrolled in this study, including 12 males and 19 females, aged 26-63 years, with a mean age of 45.8 years. There were 20 cases of urinal irritation symptoms, including serious influence on normal life in 3 cases, dysuria in 6 cases, and pain in lower abdomen in 3 cases. The results of B-ultrasound showed that 14 cases of local mucosal thickening and 14 cases of local mucosal thickening were examined by cystoscopy and pathological biopsy. All the 31 cases of cystitis glandularis were treated with selective green laser vaporization. All the cases were not treated with cystoscopy. The cystoscopy was reexamined every 3 months in the first year and every 6 months after the operation. All patients were followed up for 6-30 months with an average of 16 months. Cystoscopy biopsy was performed to confirm the recurrence of cystoscopy if there was any abnormality. This study retrospectively analyzed the perioperative data and postoperative complications in all patients. The results of cystoscopy and pathological biopsy demonstrated the safety and effectiveness of the operation. Results all the 31 cases were successfully completed, the mean operation time was 32.1min and 31 cases had no obvious bleeding. The symptoms of urinal urgency, urinal pain, frequent urination and lower abdominal pain were all disappeared, and there were no postoperative complications. The average hospital stay was 6.7 days after operation. After 6-30 months follow-up, 27 cases were cured, 3 cases improved and 1 case recurred, none case became cancerous. Conclusion: simple green laser vaporization is an effective method for the treatment of cystitis glandularis with the advantages of high cure rate, short operation time and quick postoperative recovery, and only green laser vaporization is safe in the treatment of cystitis glandularis with fewer postoperative complications.
