Klotho在IgA肾病患者血清及尿液中的表达水平及临床意义

【摘要】:目的:研究Klotho蛋白在原发性IgA肾病(IgAN)患者血清及尿液中的表达水平及临床意义。方法:选取2013年到2016年间24例在我院肾病学科住院,做肾穿刺检查后诊断为原发性IgAN患者为研究对象,其中男15例,女9例,平均年龄49.13± 15.76岁。再选取我院10例健康体检者作为对照组,其中男6例,女4例,平均年龄为44.80±13.61岁。对健康对照者和IgAN患者的血清、尿液分别采用酶联免疫吸附试验(ELISA)法检测Klotho的表达水平。与此同时,收集此次研究对象的一般情况,包括收缩压(SBP)、舒张压(DBP)、身体质量指数(BMI),再收集生化检验中的总胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、尿素氮(BUN)、血肌酐(CREA)、血尿酸(UA)。本次数据采用SPSS23.0处理各数值的表示用均数±标准差,两组间差异性的比较使用独立样本t检验,Pearson相关性分析Klotho蛋白与临床数据间的相关性。结果:1.IgAN组中SBP、DBP、ALB、CHO、TG、LDI-C高于对照组,有显著性差异(P0.01)。2.IgAN组血清和尿液Klotho 水平与健康对照组相比较降低,差异有统计学意义(P0.05)。3.IgAN 组患者血清 Klotho 水平与 DBP(r=0.471,P=0.020),CHO(1=0.483,P =0.017),LDL-C(r=0.526,?=0.008)呈负相关。结论:1.IgAN患者血清及尿液中Klotho 水平较健康对照组降低,提示Klotho的水平下降与IgAN的发展有关。Klotho水平降低可能通过激活RAS系统、引起炎症反应、损伤肾小球内皮细胞、促进肾脏纤维化等参与疾病的进展。2.IgAN患者血清中Klotho水平与DBP、CHO、LDL-C呈负相关,提示血清Klotho水平越高,高血压和高血脂的程度越减轻,血压和血脂可能在IgAN发展中起一定作用。其作用机制可能是Klotho通过直接分解脂肪细胞、减少脂质沉积、减轻血管钙化,还可直接激NO分泌、舒张血管,进而减轻肾小球内皮细胞损伤,延缓IgAN发展。
[Abstract]:Objective: to study the expression and clinical significance of Klotho protein in serum and urine of patients with primary IgA nephropathy.Methods: from 2013 to 2016, 24 patients (15 males and 9 females, mean age 49.13 卤15.76 years) with primary IgAN diagnosed by renal puncture were selected.Ten healthy persons in our hospital were selected as the control group, including 6 males and 4 females, with an average age of 44.80 卤13.61 years.The expression of Klotho in serum and urine of healthy controls and patients with IgAN were detected by Elisa.At the same time, collecting the general information of the subjects of this study,These include SBP, DBP, BMI, TGG, LDL-C0, HDL-CU, bun, creatinine creatinine creatinine (creatinine), serum uric acid (UAA).The difference between the two groups was compared using the independent sample t-test Pearson correlation analysis of the correlation between Klotho protein and clinical data.Results 1. The serum and urine Klotho levels in the IgAN group were significantly lower than those in the healthy control group. There was a negative correlation between the serum Klotho level and the serum Klotho level in the DBPr0.471P0.020 (P 0.017) LDL-C0. 5260. 008) group (P < 0. 017), and there was a negative correlation between the serum Klotho level and the serum Klotho level in the IgAN group (P = 0. 471, P = 0. 483P = 0. 017, P = 0. 483, P < 0. 05, P = 0. 008). The serum Klotho levels in the IgAN group were significantly lower than those in the healthy control group (P = 0. 05, P = 0. 05, P = 0. 05, P < 0. 05).Conclusion 1. The levels of Klotho in serum and urine of IgAN patients are lower than those of healthy controls, suggesting that the decrease of Klotho level is related to the development of IgAN. The decrease of Klotho level may cause inflammatory reaction and damage glomerular endothelial cells by activating RAS system.2. The level of serum Klotho in IgAN patients is negatively correlated with LDL-C, suggesting that the higher the level of serum Klotho, the lower the degree of hypertension and hyperlipidemia. Blood pressure and blood lipid may play a role in the development of IgAN.The mechanism may be that Klotho can directly decompose adipocytes, reduce lipid deposition, reduce vascular calcification, stimulate the secretion of no directly, relax blood vessels, and then alleviate the injury of glomerular endothelial cells and delay the development of IgAN.
