趋化因子受体4在肾透明细胞癌组织中的表达及临床意义

【摘要】：目的：探讨趋化因子受体4(CC chemokine receptor4, CCR4)在肾透明细胞癌(clear cell renal cell carcinoma, ccRCC)组织中的表达及其与临床病理特征和预后之间的关系。方法：应用免疫组织化学法检测53例ccRCC和11例正常肾脏组织中CCR4的表达。结果：肾癌组织中CCR4表达阳性率为52.83%(28/53),正常肾脏组织中CCR4阳性率为18.2%(2/11),差异有统计学意义(χ2=4.392, P=.036)。CCR4表达与肿瘤分期(P=0.009),淋巴结转移(P=0.003)及远处转移(P=0.031)相关。CCR4是ccRCC患者发生淋巴结转移唯一独立的影响因素(P=0.014).总生存时间的影响因素分析,单因素分析显示CCR4、肿瘤分期、淋巴结转移与ccRCC患者总生存时间(overall survival, OS)相关,多因素分析显示CCR4(P=0.007)是ccRCC患者OS的唯一独立预测指标。Kaplan-Meier曲线提示CCR4高表达组的ccRCC患者OS短于CCR4高表达组(P=0.010)。结论：CCR4与肾透明细胞癌的恶性生物学行为有关,可能成为判断ccRCC预后指标及潜在治疗分子靶点。
[Abstract]:Objective: to investigate the expression of chemokine receptor 4 (CC chemokine receptor4, CCR4) in renal clear cell carcinoma (clear cell renal cell carcinoma, ccRCC) and its relationship with clinicopathological features and prognosis. Methods: immunohistochemical method was used to detect the expression of CCR4 in 53 cases of ccRCC and 11 cases of normal kidney. Results: the positive rate of CCR4 expression was 52.83% (28 / 53) in renal cell carcinoma and 18.2% (2 / 11) in normal kidney tissue. CCR4 expression was associated with tumor stage (P0. 009), lymph node metastasis (P0. 003) and distant metastasis (P0. 031). CCR4 was the only independent factor affecting lymph node metastasis in ccRCC patients (P0. 014). Univariate analysis showed that CCR4, tumor staging and lymph node metastasis were correlated with total survival time (overall survival, OS) of patients with ccRCC. Multivariate analysis showed that CCR4 (P0. 007) was the only independent predictor of OS in ccRCC patients. The Kaplan-Meier curve suggested that the OS of ccRCC patients with high expression of CCR4 was shorter than that of high expression of CCR4 (P0. 010). Conclusion: CCR4 is related to the malignant biological behavior of renal clear cell carcinoma and may be a prognostic index and a potential molecular target for the treatment of ccRCC.
