肌肉骨骼超声在评估类风湿关节炎病情活动中的应用

【摘要】：目的:将肌肉骨骼超声与DAS28评分及增强MRI进行比较,探讨肌肉骨骼超声在评估类风湿关节炎患者病情活动度中的应用价值。方法:选取2015年11月~2016年11月就诊于新疆维吾尔自治区人民医院风湿免疫科,符合1987年ACR诊断标准的RA患者35例(男5,女30)。上述患者均完善双手增强MRI检查、DAS28评分、骨骼肌肉超声检查,记录数据并对其进行统计学分析。结果:(1)35名RA患者中,共有33人DAS28评分2.6;(2)MSUS和MRI分别对35名患者的右腕关节及左腕关节的检查结果差异的比较无统计学意义(P0.05),两种检查方法的一致性检验系数К=0.759,К=0.825,P=0.000,说明这两种检查方法的一致性检验有统计学意义(P0.001)。(3)MSUS与DAS28评分之间存在显著的相关性(P0.001)。结论:骨骼肌肉超声结果与增强MRI结果有较强一致性,与DAS28评分有较强一致性,在评估RA患者病情活动度中有应用价值。
[Abstract]:Objective: to compare musculoskeletal ultrasound with DAS28 score and enhanced MRI. To explore the value of musculoskeletal ultrasound in evaluating the disease activity of patients with rheumatoid arthritis. Selected from November 2015 to November 2016 to visit the people's Hospital of Xinjiang Uygur Autonomous region rheumatism immune department. In 1987, 35 RA patients (5 males and 30 females) met the diagnostic criteria of ACR. All the above patients were assessed with enhanced MRI and skeletal muscle ultrasound. The data were recorded and analyzed statistically. Results 33 of 35 RA patients were assessed with DAS28 score of 2.6; There was no significant difference between MSUS and MRI in the examination of right wrist joint and left wrist joint in 35 patients (P 0.05). The consistency coefficient of the two methods was 0.759 and 0.825% respectively. The results showed that there was a significant correlation between MSUS and DAS28 score (P 0.001). Conclusion: the results of skeletal muscle ultrasound are consistent with those of enhanced MRI. There is strong consistency with DAS28 score, and it is valuable in evaluating the disease activity of RA patients.
