PFNA、LISS治疗老年骨质疏松性股骨粗隆间骨折效果比较

【摘要】：目的比较股骨近端防旋髓内钉(PFNA)与微创内固定系统(LISS)治疗老年骨质疏松性股骨粗隆间骨折的临床疗效。方法选择老年骨质疏松性股骨粗隆间骨折患者96例,按随机数字表法分为PFNA组48例、LISS组48例。PFNA组、LISS组分别采用PFNA、LISS方法行内固定治疗。记录两组手术时间、术中出血量,统计术后1个月并发症例数。骨性愈合时间采用X线片检查评价。于术后6个月采用髋关节Harris评分评价髋关节功能,以Harris评分评价临床疗效。结果 PFNA组手术时间、术中出血量及骨性愈合时间均少于LISS组(P均0.05)。LISS组术后1、6个月Harris评分分别为(53.96±3.47)、(69.74±6.23)分,PFNA组分别为(66.71±4.49)、(88.11±5.17)分,PFNA组术后1、6个月Harris评分均高于LISS组(P均0.05)。LISS组优28例、良15例、可3例、差2例、优良率为89.58%(33/48),PFNA组分别为31、14、2、1例及93.75%(45/48),两组临床疗效差异无统计学意义(P0.05)。两组并发症发生率差异无统计学意义(P0.05)。结论与LISS相比,老年骨质疏松性股骨粗隆间骨折应用PFNA行内固定的手术时间及骨性愈合时间均较短,术中出血量较少,髋功能恢复较好。
[Abstract]:Objective to compare the clinical effects of PFNA- and minimally invasive internal fixation system (LISS) in the treatment of senile osteoporotic intertrochanteric femoral fractures. Methods 96 elderly patients with osteoporotic femoral intertrochanteric fractures were selected. According to the method of random digital table, 48 cases of PFNA group were divided into two groups: 48 cases of Liss group. 48 cases of Liss group in PFNA group were treated with PFNA-Liss method respectively. The time of operation and the amount of blood lost during operation were recorded in the two groups. The bone healing time was evaluated by X-ray radiography. The hip function was evaluated by hip Harris score at 6 months after operation, and the clinical efficacy was evaluated by Harris score. Results the operative time was evaluated in PFNA group. The intraoperative bleeding volume and bone healing time were less than those in LISS group (P 0.05N 路Liss group) 1 and 6 months after operation, respectively. The Harris scores in PFNA group were 53.96 卤3.47 and 69.74 卤6.23 respectively. The scores of Harris in PFNA group were 66.71 卤4.49nil 卤88.11 卤5.17, respectively. The Harris scores at 6 months were significantly higher than those in LISS group (P 0.05), good in 15 cases, fair in 3 cases, and poor in 2 cases. The excellent and good rates were 89.58% 33 / 48% PFNA and 93.75% 45 / 48% PFNA, respectively. There was no significant difference in clinical efficacy between the two groups (P 0.05). There was no significant difference in the incidence of complications between the two groups (P 0.05). Conclusion compared with LISS, there is no significant difference in the incidence of complications between the two groups. In elderly patients with osteoporotic intertrochanteric fracture, the operative time and bone healing time of internal fixation with PFNA were shorter, the amount of intraoperative bleeding was less, and the hip function was better.
