扩散峰度成像技术对慢性高原病患者与正常人大脑灰白质微结构的对照研究

【摘要】:目的:应用扩散峰度成像(DKI)研究慢性高原病(CMS)患者与正常人大脑灰白质微结构的差异。方法:收集21例CMS患者为病例组,20例健康志愿者为正常对照组,均行常规头颅MR及DKI序列扫描,分别测量两组大脑灰白质各感兴趣区(ROI)内的平均峰度(MK)值、径向峰度(RK)值及峰度各向异性(KA)值,分别统计各组同一部位左右两侧ROI参数,符合正态分布,采用配对样本t检验;统计两组间同侧同一部位ROI参数,符合正态分布,采用两独立样本t检验。结果:同组比较,CMS组两侧内囊前肢KA值及两侧尾状核头、丘脑MK值、RK值存在侧别差;对照组两侧内囊前肢MK值、KA值及丘脑MK值、RK值存在侧别差;两组的右侧内囊前肢的KA值较对侧减低,右侧丘脑MK及RK值较对侧增高。两组间对比,胼胝体膝部RK值有显著性,CMS组低于对照组;胼胝体压部KA值、左侧内囊前肢MK值、左侧内囊后肢RK值、右侧尾状核头MK值、双侧豆状核MK值及右侧丘脑MK值、KA值差异有显著性,CMS组高于对照组。结论:高海拔地区正常人及CMS患者左右大脑半球微结构并非完全对称,部分脑区存在差异。CMS患者脑组织对高原低氧环境敏感性及反应性不一,胼胝体、基底节区及丘脑与正常组均有不同程度差异性。
[Abstract]:Objective: to study the difference of brain gray matter microstructure between patients with chronic altitude disease (CMSS) and normal subjects by using diffusion kurtosis imaging (DKI). Methods: 21 CMS patients were collected as case group and 20 healthy volunteers as control group. The mean kurtosis, radial kurtosis, kurtosis and kurtosis anisotropic kurtosis were measured by routine cranial Mr and DKI sequences respectively. The ROI parameters of the left and right sides of each group were counted. According to normal distribution, paired sample t test was used, ROI parameters in the same part of the same side between two groups were calculated, and two independent samples t test were used. Results: the Ka value of anterior limb of bilateral internal capsule and the head of caudate nucleus were compared in the same group. The MK value of the thalamus and the RK value of the anterior limb of the internal capsule in the control group were different from those of the control group, and the Ka value of the right anterior limb of the internal capsule and the MK value of the thalamus of the two groups were lower than those of the opposite side, and the values of MK and RK of the right thalamus were higher than those of the opposite side. The RK value of the genu of corpus callosum was significantly lower than that of the control group, the Ka value of the corpus callosum, the MK value of the anterior limb of the left internal capsule, the RK value of the posterior limb of the left internal capsule, and the MK value of the head of the right caudate nucleus. The MK value of bilateral lenticular nucleus and the right thalamus MK value were significantly higher in CMS group than in control group. Conclusion: the microstructures of left and right hemispheres of normal subjects and CMS patients at high altitude are not completely symmetrical. There were differences in some brain regions. The sensitivity and responsiveness of brain tissue to high altitude hypoxic environment were different in CMS patients. The corpus callosum basal ganglia and thalamus were different to different degrees from normal group.
