不同剂量人脐带间充质干细胞移植治疗MS效果观察

【摘要】:目的对比观察不同剂量人脐带间充质干细胞(hUC-MSC)移植治疗多发性硬化(MS)的临床效果。方法将60例MS患者随机均分为A、B、C、D组。从足月健康新生儿脐带中提取hUC-MSC;各组均采用BEAM方案(卡莫司汀、替尼泊苷、阿糖胞苷、美法仑)进行预处理。A、B、C组采用腰椎穿刺术经蛛网膜下腔分别注射hUCMSC 1.5×10~6/kg、7.5×10~6/kg、15×10~6/kg,D组给予相同体积生理盐水。每2周移植1次,共移植3次。治疗前后采用扩展致残状况评分量表(EDSS)对患者神经功能缺损程度进行评分,采用MR检查头颅、脊髓病灶数量及病灶体积变化,统计患者治疗期间发生的不良反应。结果与治疗前比较,治疗后A、B、C组EDSS较D组低(P均0.05),且C组B组A组,两两比较差异有统计学意义(P均0.05)。治疗后A、B、C组病灶数目及体积均较D组小(P均0.05),且C组B组A组,两两比较差异有统计学意义(P均0.05)。治疗过程中B、D组各出现1例低等发热,经对症处理后均于3 d内恢复正常。结论在常规抗炎治疗基础上加hUC-MSC移植治疗能改善MS患者的临床症状;且在≤15×106/kg范围内,移植剂量越高治疗效果越佳。
