孤儿核受体NR4A1在2型糖尿病患者外周血单个核细胞的表达及意义

【摘要】:目的探讨孤儿核受体NR4A1在2型糖尿病(T2DM)患者外周血单个核细胞(PBMCs)的表达及意义。方法选取就诊的34例健康受试者和30例初诊T2DM患者,进行体格检查,包括身高、体重、血压,计算体质指数(BMI);实验室检查:空腹血糖(FBG)、空腹胰岛素(Fins)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、超敏C反应蛋白(hs-CRP),计算HOMA-IR。采集外周血2~3 m L,收集血浆,用于检测游离脂肪酸(FFAs)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6),并采用密度梯度离心法获取PBMCs,real-time PCR检测NR4A1 mRNA在健康人群和初诊T2DM患者PBMCs中分布情况,并对NR4A1 mRNA相对表达水平和HOMA-IR、FFAs、TNF-α、IL-6进行相关性分析。结果两组人群在年龄、性别、身高、血压、血丙氨酸氨基转移酶、血尿素氮、血肌酐方面,差异无统计学意义(P0.05);与健康受试者比较,初诊T2DM患者的PBMCs的NR4A1 mRNA相对表达水平增高,此外T2DM患者的BMI、FBG、Fins、HOMAIR、TG、TC、LDL-C、hs-CRP、FFAs、TNF-α和IL-6水平亦明显增高,血HDL-C水平则降低;T2DM患者的外周血白细胞计数及其分类计数与健康受试者差异无统计学意义(P0.05);Pearson's相关分析显示,NR4A1 mRNA相对表达水平与HOMA-IR(r=0.761,P0.01)、FFAs(r=0.560,P0.01)、TNF-α(r=0.697,P0.01)和IL-6(r=0.796,P0.01)均呈正相关。同时,评价胰岛素抵抗程度的HOMA-IR也与FFAs(r=0.513,P0.01)、TNF-α(r=0.728,P0.01)和IL-6(r=0.590,P0.01)呈正相关。此外,显著正相关也存在于FFAs和TNF-α(r=0.475,P0.01)、IL-6(r=0.402,P0.01)之间。结论 NR4A1在初诊T2DM患者PBMCs中表达增高,且与HOMA-IR、FFAs、TNF-α、和IL-6均呈正相关关系。
[Abstract]:Objective to investigate the expression and significance of orphan nuclear receptor (NR4A1) in peripheral blood mononuclear cells (PBMCs) from patients with type 2 diabetes mellitus (T2DM). Methods 34 healthy subjects and 30 newly diagnosed T2DM patients were selected for physical examination, including height, weight, blood pressure. Calculated body mass index (BMI); laboratory examination: fasting blood glucose FBGG, fasting insulin Finsn, triacylglycerol TGG, total cholesterol TCU, LDL-Cn, HDL-CU, HDL-CU, hypersensitive C-reactive protein hs-CRPU, HOMA-IRN were calculated in all patients with high density lipoprotein cholesterol (HDL-C), and hypersensitive C-reactive protein (HDL-CRPU). Peripheral blood samples were collected at 2mL, and plasma samples were collected for the detection of free fatty acid (FFA), tumor necrosis factor- 伪 (TNF- 伪), interleukin-6 (IL-6), and the distribution of NR4A1 mRNA in PBMCs of healthy population and newly diagnosed T2DM patients was detected by real-time PCR obtained by density gradient centrifugation. The correlation between the relative expression of NR4A1 mRNA and Homa-IRFAsT TNF- 伪-6 was analyzed. Results there was no significant difference in age, sex, height, blood pressure, serum alanine aminotransferase, blood urea nitrogen and serum creatinine between the two groups. In addition, the levels of IL-6 and TNF- 伪 in T2DM patients were also significantly higher than those in patients with T2DM, and the levels of LDL-Cnhs-CRP FFAsTNF- 伪 and TNF- 伪 were also significantly increased in patients with T2DM, and the levels of TNF- 伪 and TNF- 伪 were also significantly increased in patients with T2DM. There was no significant difference in peripheral blood white blood cell count and its classification between patients with T2DM and healthy subjects. The correlation analysis showed that the relative expression level of NR4A1 mRNA was positively correlated with that of HOMA-IRR 0.761P0.01FFASS 0.560P0.01TNF- 伪 TNF- 伪 0.796P0.01). At the same time, HOMA-IR for evaluating the degree of insulin resistance was also positively correlated with TNF- 伪 -0.728 (P0.01) and IL-6 (0.590) P0.01). In addition, a significant positive correlation was also found between FFAs and TNF- 伪 (0.475P0.01P0.01P0.01). Conclusion the expression of NR4A1 in PBMCs of newly diagnosed T2DM patients is increased, and it is positively correlated with HOMA-IRFFAsTNF- 伪 and IL-6.
