2型糖尿病患者血清PLGF水平与颈动脉粥样硬化的相关性研究

【摘要】:目的:探讨2型糖尿病(T2DM)患者血清胎盘生长因子(PLGF)与颈动脉粥样硬化(AS)的相关性,通过分析PLGF与糖脂代谢及炎症因子的相关性,为阐明其在2型糖尿病患者颈动脉粥样硬化过程中所起的作用提供一定的理论依据,探讨其作为糖尿病颈动脉粥样硬化及其它心血管并发症早期诊断、干预及治疗靶点的可能性。方法:根据彩色多普勒超声仪检查双侧颈动脉的结果,将53例2型糖尿病患者分为T2DM无颈动脉粥样斑块组(T2DM+NAS组,27例)、T2DM合并颈动脉粥样斑块组(T2DM+AS组,26例);同期门诊健康体检者26例为正常对照组(NC组)。测量血压、身高、体重、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、胰岛素(FINS)、糖化血红蛋白(Hb Alc)、超敏C反应蛋白(hs CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6),并计算体重指数(BMI)、胰岛素抵抗指数(HOMA-IR)。采用双抗体夹心酶联免疫吸附实验(ELISA)检测所有受试者血清PLGF浓度。结果:(1)三组比较,T2DM+NAS组、T2DM+AS组血清PLGF水平(45.35±13.00,59.24±11.05)pg/m L均显著高于NC组(27.30±10.22)pg/m L(P0.01),T2DM+AS组血清PLGF水平显著高于T2DM+NAS组(P0.01)。(2)三组血清PLGF水平与BMI、SBP、FPG、TC、TG、LDL-C、Hb A1c、hs CRP、TNF-α、IL-6呈正相关(r分别为0.311、0.266、0.455、0.363、0.260、0.395、0.575、0.396、0.411、0.449,P0.05);T2DM患者血清PLGF水平与TC、LDL-C、hs CRP、IL-6呈显著正相关(r分别为0.351、0.387、0.272、0.300,P0.05)。(3)多元线性逐步回归结果显示:LDL-C、IL-6为血清PLGF水平的独立影响因素(β=7.226、1.537,P0.05)。(4)Logistic回归分析显示PLGF为颈动脉粥样硬化的风险预测因子。结论:(1)PLGF在颈动脉正常的T2DM患者血清中表达已增多,在T2DM合并颈动脉粥样斑块的患者血清中表达进一步增多,提示PLGF可能参与了2型糖尿病患者颈动脉粥样斑块的早期形成,促进了动脉粥样硬化的发生、发展,或可作为2型糖尿病患者预测颈动脉粥样硬化的一个早期指标。(2)相关分析显示PLGF水平与炎症指标hs CRP、TNF-α和IL-6呈正相关,IL-6为PLGF水平独立影响因素,提示PLGF可能通过参与炎症反应促进2型糖尿病颈动脉粥样硬化的发生发展。(3)血清PLGF水平与BMI、血压、血糖、血脂均密切相关,良好控制血压、血糖、血脂,减轻体重或可使T2DM患者血清PLGF水平下降,从而延缓颈动脉粥样硬化发展。
[Abstract]:Objective: to investigate the relationship between serum placental growth factor (PLGF) and carotid atherosclerosis (ASA) in patients with type 2 diabetes mellitus (T2DM). To provide a theoretical basis for elucidating its role in the process of carotid atherosclerosis in patients with type 2 diabetes, and to explore its role as an early diagnosis of carotid atherosclerosis and other cardiovascular complications in patients with type 2 diabetes mellitus. The possibility of intervention and therapeutic targets. Methods: according to the results of color Doppler ultrasound examination of bilateral carotid artery, 53 patients with type 2 diabetes mellitus were divided into two groups: T2DM without carotid atherosclerotic plaque group (n = 27), T2DM NAS group (n = 27) with T2DM combined with carotid atherosclerotic plaque group (n = 26) and normal control group (n = 26). Measuring blood pressure, height, weight, Fasting blood glucose (FPG), total cholesterol (TCU), triglyceride (TGN), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), insulin (FINSN), glycosylated hemoglobin (HbAlcn), hypersensitive C-reactive protein (hsCRPN), tumor necrosis factor- 伪 (TNF- 伪) and interleukin-6 (IL-6). Body mass index (BMI) and insulin resistance index (ISI) were calculated.
