基于慢性病管理模型的社区代谢综合征干预研究

【摘要】：一、研究目的通过文献研究国外慢性病管理方法,引进慢性病管理模型,通过类实验研究,将该模型应用于社区代谢综合征患者,探索改进慢性病管理策略,提高和改善患者健康结局。二、研究方法1.文献研究法:通过查阅国外关于慢性病管理方法的相关文献,介绍慢性病管理模型。2.试验研究法:方便抽取浙江省杭州市余杭区乔司街道的三个社区,采用类实验研究方法进行研究设计,将两个社区设置为对照组研究对象所在区域,一个社区设置为实验组研究对象所在区域。对照组采取常规社区慢性病管理方法,实验组采取慢性病管理模型,分别对代谢综合征患者开展为期100天的健康管理。使用自行设计的一般资料调查表、自我效能量表、健康状况调查问卷进行资料收集,并采集血标本进行生化指标分析。三、研究结果干预后,实验组研究对象的空腹血糖值下降(P0.05),甘油三脂下降(P0.01),自我效能得分升高(P0.001),在生命质量方面,生理功能、躯体疼痛、总体健康、活力、社会功能、精神健康维度得分均提高,差异具有统计学意义。上述指标与对照组进行组间对比,差异均具有统计学意义。对照组干预前后进行组内对比,差异无统计学意义。四、研究结论慢性病管理模型能够部分提高和改善社区代谢综合征患者的健康结局,更多的效果需要更长时间的干预来确认。
[Abstract]:Objective: to study the methods of chronic disease management in foreign countries, introduce chronic disease management model, apply the model to metabolic syndrome patients in community, and explore how to improve the management strategy of chronic disease. Improve and improve the patient's healthy outcome. Second, research method 1. Literature research method: through consulting the relevant literature about chronic disease management methods abroad, introduce chronic disease management model. 2. 2. Experimental research method: three communities of Joss street in Yuhang District, Hangzhou City, Zhejiang Province were selected conveniently, and the research design was carried out by using the similar experimental research method. The two communities were set up as the study object area of the control group. A community was set up as the study area of the experimental group. The control group adopted the routine community chronic disease management method, the experimental group adopted the chronic disease management model, the metabolic syndrome patients were carried out 100 days of health management. The self-designed general information questionnaire, self-efficacy scale and health status questionnaire were used to collect data, and blood samples were collected for biochemical analysis. 3. After intervention, fasting blood glucose (FBG), triglyceride (P0.01), self-efficacy (P0.001), physical function, physical pain, general health, vitality, social function were decreased in the experimental group. The scores of mental health were all improved, and the difference was statistically significant. The above indexes were compared with the control group, the difference was statistically significant. The contrast before and after intervention in the control group showed no statistical significance. Fourth, the study concluded that chronic disease management model can partially improve and improve the healthy outcome of patients with metabolic syndrome in community, more effects need longer intervention to confirm.
