皮肤外伤诱发肿胀性红斑狼疮1例

【摘要】：患者女,56岁。左侧面部红斑、肿胀伴疼痛1.5年。发病前曾被仙人球刺伤。抗核抗体颗粒型1∶100阳性。皮损直接免疫荧光检查阴性,皮损组织病理示:表皮基底细胞液化变性,血管及附属器周围淋巴细胞浸润,阿新蓝染色真皮胶原纤维间可见大量黏蛋白沉积。诊断:肿胀性红斑狼疮。予甲泼尼龙(逐渐减量至停药,共计1.5年)、羟氯喹(1年)、雷公藤多苷片(1个月)口服,并予他克莫司软膏外搽1年,皮损已消退,未见遗留瘢痕。
[Abstract]:The patient was 56 years old. Left face erythema, swelling with pain 1.5 years. He was stabbed by a cactus before the onset of the disease. The antinuclear antibody granule type was 1: 100 positive. The lesions were negative by direct immunofluorescence. The pathological findings of the lesions were as follows: liquefaction of basal cells of epidermis, infiltration of lymphocytes around blood vessels and appendages, and a large amount of mucin deposition between collagen fibers of dermis stained with acinyl blue. Diagnosis: swollen lupus erythematosus. Methylprednisolone (1.5 years), hydroxychloroquine (1 year), Tripterygium wilfordii polyglycoside (1 month) and tacrolimus ointment were given for 1 year.
