抗氧化能力在类风湿性关节炎患者中的临床意义及与疾病活动的关系

【摘要】:目的: 通过检测类风湿性关节炎(RA)患者血清中抗氧化标志物总抗氧化状态(TAS)和超氧化物歧化酶(SOD)的水平,分析TAS和SOD与RA患者临床以及实验室疾病活动性指标的相关性,探讨抗氧化能力在RA发病机制及疾病进展中的作用。 方法: 收集2013年3月至2014年12月在吉林大学第一医院风湿免疫科住院并且临床资料完整的RA患者91例作为病例组,根据RA疾病活动性评分(DAS28)又将其分为低疾病活动组(38人,DAS28≤3.2,简称低活动组)和中高度疾病活动组(53人,DAS283.2,简称中高活动组)。记录RA患者相关临床指标如疾病持续时间、晨僵时间、压痛关节数、肿胀关节数、疾病活动度评分以及实验室指标如白细胞、红细胞、血小板、红细胞沉降率、C-反应蛋白、类风湿因子、抗环瓜氨酸多肽抗体。同期收集来自于吉林大学第一医院体检中心健康体检者的70例作为健康对照组,记录实验室指标如白细胞、红细胞、血小板、红细胞沉降率、C-反应蛋白、类风湿因子、抗环瓜氨酸多肽抗体。检测血清TAS和SOD水平,并分析其与RA各项临床及实验室指标的相关性。结果用SPSS17.0进行统计学分析。 结果: 1.RA组TAS、SOD浓度显著低于健康对照组,ESR、CRP、RF、Anti-CCP浓度显著高于健康对照组,差异具有统计学意义(P0.05)。 2.RA中高活动组TAS、SOD、RBC水平显著低于低活动组及健康对照组,WBC、PLT、ESR、CRP、RF、Anti-CCP水平显著高于低活动组及健康对照组,晨僵时间、TJC、SJC、DAS28显著高于低活动组,差异有统计学意义(P0.05)。低活动组TAS、SOD水平显著低于健康对照组,WBC、PLT、ESR、CRP、RF、Anti-CCP水平显著高于健康对照组,差异有统计学意义(P0.05)。 3.RA患者血清TAS水平与RF、TJC、SJC、CRP、ESR、DAS28呈负相关(P0.05),与WBC、Anti-CCP无显著相关性(P0.05)。其中TAS水平与DAS28、TJC相关系数最高,分别为-0.588、-0.572;与CRP、ESR相关系数次之分别为-0.500、-0.421,,与其他血液学参数如WBC、RBC、PLT相关系数较低,分别为-0.197、0.081、-0.228。 4.RA患者血清SOD水平与RF、TJC、SJC、CRP、ESR、DAS28呈负相关(P0.05),与WBC、Anti-CCP无显著相关性(P0.05)。其中SOD水平与DAS28、TJC相关系数最高,分别为-0.677、-0.607;与CRP、ESR相关系数次之分别为-0.591、-0.492,与其他血液学参数如WBC、RBC、PLT相关系数较低,分别为-0.067、0.372、-0.132。 5.RA血清SOD水平与TAS水平存在正相关(r=0.499,P0.05),有统计学意义。 结论: 1.氧化应激参与RA的致病过程。 2.RA患者抗氧化能力减弱,血清抗氧化标志物SOD和TAS可用于RA辅助诊断和疗效观察。 3.血清中TAS、SOD水平与疾病活动性相关,可作为评价RA疾病活动性的指标之一。 4.TAS、SOD水平对研究RA疾病的发生发展机制有辅助价值。
