老年糖尿病足溃疡患者营养状况调查及其影响因素分析

【摘要】:目的调查老年糖尿病足溃疡患者的营养状况并分析其影响因素。方法采用方便抽样法,选取老年糖尿病足溃疡患者182例,应用微型营养评估量表对患者进行营养状况评估,收集患者的相关临床资料,检测糖化血红蛋白、血红蛋白、血清白蛋白和C反应蛋白浓度。结果 182例患者的微型营养评分为(16.36±3.68)分,其中121例(66.48%)发生营养不良,61例(33.52%)存在营养不良风险。不同年龄、糖尿病病程、膳食摄入情况、糖化血红蛋白、感染、C反应蛋白、足溃疡分级的老年糖尿病足溃疡患者的微型营养评估量表得分进行比较,差异具有统计学意义(P0.05);足溃疡Wagner 3级以上患者的营养不良发生率、C反应蛋白浓度高于足溃疡Wagner 1~2级的患者,血红蛋白、血清白蛋白浓度低于足溃疡Wagner 1~2级的患者,两者比较,差异有统计学意义(P0.05)。结论老年糖尿病足溃疡患者营养不良发生率较高,并与炎症状态密切相关,医护人员应及时提供针对性的干预措施,改善患者的营养状况。
[Abstract]:Objective to investigate the nutritional status of elderly diabetic foot ulcer patients and analyze its influencing factors. Methods 182 elderly patients with diabetic foot ulcers were selected by convenient sampling method. The nutritional status of the patients was evaluated by mini-nutritional assessment scale. The clinical data of the patients were collected and the glycosylated hemoglobin (HbA1c) was detected. Serum albumin and C-reactive protein concentration. Results the mini-nutritional score of 182 patients was (16.36 卤3.68), of which 121 (66.48%) suffered from malnutrition and 61 (33.52%) were at risk of malnutrition. The scores of mini-scale were compared among different ages, diabetes duration, dietary intake, glycosylated hemoglobin, infection, C-reactive protein, foot ulcer grade in elderly patients with diabetic foot ulcer. The difference was statistically significant (P0.05); The incidence of malnutrition in patients with foot ulcers above Wagner grade 3, C-reactive protein concentrations higher than those in patients with foot ulcers grade Wagner 1 / 2, hemoglobin and serum albumin levels lower than those in patients with foot ulcers grade Wagner 1 / 2, were compared between the two groups. The difference was statistically significant (P0.05). Conclusion the incidence of malnutrition in elderly patients with diabetic foot ulcer is high and closely related to the inflammatory state. Medical staff should provide timely intervention measures to improve the nutritional status of the patients.
