老年类风湿关节炎患者颈椎失稳的影响因素

【摘要】：目的探讨老年类风湿关节炎(RA)患者颈椎失稳的影响因素。方法老年RA患者394例,依据患者是否发生颈椎失稳分为颈椎失稳组(n=52)和对照组(n=342),对入组患者的临床资料进行单因素分析和多因素Logistic回归分析。结果颈椎失稳患者多为寰枢椎前后半脱位,下颈椎半脱位患者发生率最低。颈椎失稳组体重指数(BMI)低、病程长、吸烟史长、外周关节骨破坏多、关节肿胀个数多、关节压痛个数多、使用生物制剂、长期使用激素、健康评估问卷残疾指数(HAQ-DI)高、DAS28高、类风湿因子(RF)多、C反应蛋白(CRP)多的比例较对照组更高(P0.05)。病程长、外周关节骨破坏多、长期使用激素是老年RA患者颈椎失稳的独立危险因素(P0.05)。结论病程长、外周关节骨破坏多、长期使用激素是老年RA患者颈椎失稳的独立危险因素,临床上应重视高危患者,给予早期干预和有效治疗。
[Abstract]:Objective to investigate the influencing factors of cervical instability in elderly patients with rheumatoid arthritis (RA). Methods 394 elderly patients with RA were divided into two groups according to whether the patients had cervical instability (n = 52) and control group (n = 342). The clinical data of the patients in the group were analyzed by univariate analysis and multi-factor Logistic regression analysis. Results most of the patients with cervical instability were anterior and posterior subluxation of atlantoaxial vertebra, and the incidence of lower cervical subluxation was the lowest. In the unstable cervical vertebra group, the body mass index (BMI) is low, the course of disease is long, the smoking history is long, the peripheral joint bone is destroyed, the number of joint swelling is more, the number of joint tenderness is more, biological preparation, long-term use of hormone, The disability index (HAQ-DI), DAS28, rheumatoid factor (RF) and C-reactive protein (CRP) were higher in the health assessment questionnaire than those in the control group (P0.05). The long-term use of hormone was the independent risk factor of cervical instability in elderly patients with RA (P0.05). Conclusion long-term use of hormone is an independent risk factor of cervical instability in elderly patients with RA. High-risk patients should be paid attention to in clinic, early intervention and effective treatment should be paid attention to.
