低谷浓度他克莫司对活体肝移植术后糖尿病发病风险的影响

【摘要】：目的探究低谷浓度他克莫司(cTAC)降低活体肝移植术后糖尿病(PTDM)发病风险的安全性与可行性。方法回顾性分析2005年6月至2014年9月,我中心符合纳入标准的158例活体肝移植患者资料。定义平均cTAC为诊断PTDM当年的cTAC平均值,或未诊断PTDM患者最近一年的cTAC平均值。运用受试者工作特征曲线(ROC曲线)确定平均cTAC临界值。结果 34例(21.5%)患者诊断PTDM。PTDM组1、5、10年移植物生存率及总体生存率均低于非PTDM组,差异有统计学意义(P0.05)。预测PTDM的平均cTAC临界值为5.9ng/mL。低cTAC组(≤5.9ng/mL)术后血脂异常、PTDM、心脑血管疾病与中重度感染发生率低于高cTAC组(5.9ng/mL),差异有统计学意义(P0.05);然而两组间急、慢性排斥反应发生率的差异无统计学意义。结论活体肝移植术后6个月后,维持cTAC≤5.9ng/mL是安全有益的。
[Abstract]:Objective to investigate the safety and feasibility of low concentration tacrolimus (cTAC) in reducing the risk of diabetic (PTDM) after living donor liver transplantation (LLT). Methods from June 2005 to September 2014, 158 living-related liver transplantation patients in our center were analyzed retrospectively. The mean cTAC was defined as the mean value of cTAC in the year of diagnosis of PTDM, or the mean value of cTAC in the last year of undiagnosed PTDM. The mean cTAC critical value was determined by using the receiver operating characteristic curve (ROC curve).
