肠旁路术后GK大鼠消化吸收功能变化情况及其对血糖血脂的影响

【摘要】:【目的】研究肠旁路术(Duodenal-jejunal bypass,DJB)后非肥胖型2型糖尿病(Goto-Kakizaki,GK)大鼠消化吸收功能变化情况及其对血糖、血脂的影响。同时和改良胆胰转流术(New Billiopancreatic diversion,NBPD)组及假手术组进行对比研究,观察三组GK大鼠葡萄糖、淀粉、蛋清灌胃后血糖变化及正常饮食后,血糖血脂的变化情况,探讨外科手术治疗2型糖尿可能的机制。【方法】1.15周龄雄性GK大鼠30只,依据体重完全随机分为三组,分别行十二指肠-空肠转流术(DJB组10只)、改良胆胰转流术(NBPD组10只)、假手术组(SHAM组10只)。2.术前1周,术后1、2、4、6周检测空腹体重;术前2周,术后2、4、6周检测空腹血糖(FPG)和随机血糖(RBG)。3.术前2周术后2、4、6周,做口服糖耐量试验(OGTT)。4.术前2周及术后6周,测空腹及餐后2h血脂。5.分别于术后第42、43天,使用50%葡萄糖和红薯淀粉按照8kcal/kg的热量进行灌胃,尾静脉采血测0、30、60、120、180min血糖,并计算曲线下面积(AUC)。6.术后第46天,使用蛋清按照8kcal/kg的热量进行灌胃,然后测0,30,60,120,180min血糖值,计算出血糖曲线下面积(AUC)。【结果】1.术前,三组大鼠空腹体重差异均无统计学意义。术后1周,三组大鼠体重低于术前,差异有统计学意义。术后2、4、6周三组大鼠空腹体重差异无统计学意义。术前2周,及术后2、4、6周三组GK大鼠空腹血糖之间差异均无统计学意义。术后2、4、6周,DJB组和NBPD组随机血糖低于假手术组,术后2、6周DJB组、NBPD组和术前相比,差异有统计学意义,在术后6周,NBPD组和假手术组差异有统计学意义。2.术前2周,三组大鼠之间OGTT血糖曲线下面积差异无统计学意义,术后2、4、6周,DJB组和NBPD组OGTT血糖曲线下面积明显小于假手术组,在术后4、6周,NBPD组OGTT血糖曲线下面积和假手术之间差异有统计学意义。3.术前2周,三组大鼠之间空腹及餐后2h胆固醇,甘油三酯,游离脂肪酸无差异。在术后6周,DJB组、NBPD组空腹胆固醇、游离脂肪酸和假手术相比,差异具有统计学意义,DJB组餐后2h胆固醇、游离脂肪酸和假手术组及术前相比,差异有统计学意义。4.使用淀粉和葡萄糖灌胃后,DJB和NBPD组血糖曲线下面积都明显低于假手术组,两种物质灌胃后NBPD组血糖曲线下面积和假手术相比,差异均统计学意义。DJB组和NBPD组淀粉灌胃后30、60、120min血糖值及血糖曲线下面积均小于葡萄糖灌胃的值。5.蛋清灌胃后,三组大鼠血糖值稍微波动,血糖曲线下面积差异无统计学意义。【结论】1.肠旁路术(DJB)、改良胆胰转流术(NBPD)都能改善GK大鼠血糖。2.DJB术和NBPD术后,都改善GK大鼠血脂。3.DJB术和NBPD术后,GK大鼠消化吸收功能的变化,和胆胰液的转流有关,这可能是改善血糖血脂的主要机制之一。
[Abstract]:[Objective] To study the changes of digestive and digestive function of non-obese type 2 diabetes (Goto-Kakizaki, GK) rats after intestinal bypass (DJB) and its effect on blood sugar and blood lipid. [Methods] Thirty-one (15)-week-old male GK rats were randomly divided into three groups according to their weight, and were divided into three groups according to their weight, and were divided into three groups according to the weight of body weight. The fasting body weight was detected in the first, second, 4th and 6th week after operation. The fasting blood glucose (FPG) and the random blood sugar (RBG) were measured at 2,4 and 6 weeks after the operation.3. The fasting blood glucose (FPG) and the random blood sugar (RBG) were measured at 2,4 and 6 weeks after the operation. [Results] 1. There was no significant difference in the body weight of the three groups before and after the operation. The difference of the fasting blood glucose in the group of 2,4 and 6 weeks after the operation was not statistically significant. The difference of fasting blood glucose in the group GK rats after 2,4,6,2,4, and 6 weeks after operation was not statistically significant. The difference between the two groups,2,4,6 weeks, DJB group and NBPD group was significantly lower than that of the sham operation group. The difference of the area under the OGTT blood glucose curve of the two weeks,6 weeks,6 weeks, DJB group and NBPD group was significantly lower than that of the sham operation group, and the difference between the area under the OGTT blood glucose curve and the false operation of the NBPD group was statistically significant at 4,6 and 6 weeks after the operation. [Conclusion] 1. Enteral bypass (DJB) and modified BPD can improve the blood sugar of GK rats.2. DJB and NBPD improve the blood lipid of GK rats.3. The changes of the absorption function of GK rats after DJB and NBPD are related to the bypass of the bile and pancreatic juice, which may be one of the main mechanisms to improve blood glucose and blood lipid.
