[Abstract]:Objective to investigate the factors affecting the screening of type 2 diabetes mellitus by general practitioners, and to describe the interpretation of these factors by general practitioners and their communication with patients about the screening results. Methods semi-structured interview method based on medical record induction review was used in this study. The subjects were 20 general practitioners from the University of Michigan School of Medicine and the hospital system. The purpose of the interview was whether 134 non-diabetic patients who met the criteria of the American Diabetes Association were screened for type 2 diabetes by general practitioners. The interview was recorded, then translated into text and qualitatively analyzed. Results 63 patients were not screened for type 2 diabetes mellitus. The reason why these patients were not screened was that 49% of general practitioners thought they did not need to do it based on the results of previous screening, and 48% explained that patients were only here for counseling rather than health care tests. A total of 71 patients were screened for type 2 diabetes mellitus. The reasons for screening these patients are as follows: 49% of general practitioners think they need to do it according to the abnormal results of previous screening, 42% of general practitioners judge their needs according to the body mass of patients, and 38% of general practitioners judge their needs according to the patient's age. The correct rate of interpretation of the screening results by the general practitioner was 89%, and the proportion of the screening results told to the patients realistically was 95%. Of the 24 patients with prediabetes, 58% of the general practitioners gave advice to prevent diabetes by losing weight and increasing the amount of activity. Conclusion the previous screening results of type 2 diabetes mellitus, the types of items that general practitioners should do, the body mass and age of patients are the factors that affect the screening of type 2 diabetes mellitus by general practitioners. Generally speaking, general practitioners can correctly understand the screening results of patients and convey them to patients realistically, and its advice on the prevention of diabetes in patients with prediabetes is of evidence-based significance.
