硬膜外神经阻滞联合脊神经背根脉冲射频治疗老年带状疱疹后遗神经痛效果观察

【摘要】：目的观察硬膜外神经阻滞联合脊神经背根脉冲射频治疗老年带状疱疹后遗神经痛的临床疗效。方法将70例带状疱疹后遗神经痛老年患者随机分为观察组和对照组各35例,两组均口服加巴喷丁并行硬膜外神经阻滞,观察组加用脊神经背根脉冲射频。治疗前与治疗后4周,采用视觉模拟评分法(VAS)评价疼痛程度,分别采用焦虑自评量表(SAS)、抑郁自评量表(SDS)评价焦虑、抑郁情况,评价两组临床疗效。结果治疗前两组VAS及SDS、SAS评分比较,差异无统计学意义(P均0.05);治疗后两组VAS及SDS、SAS评分均低于治疗前,且观察组VAS及SDS、SAS评分低于对照组,差异有统计学意义(P均0.05)。观察组治疗总有效率94.29%,低于对照组的77.14%(P0.05)。结论采用硬膜外神经阻滞联合脊神经背根脉冲射频辅助治疗带状疱疹后遗神经痛,可减轻患者疼痛,间接改善其不良情绪,有助于提高临床疗效。
[Abstract]:Objective to observe the clinical effect of epidural nerve block combined with pulse radiofrequency of dorsal root of spinal nerve in the treatment of postherpetic neuralgia in elderly patients. Methods 70 cases of senile patients with postherpetic neuralgia were randomly divided into observation group (n = 35) and control group (n = 35). Both groups were given gabapentin and epidural nerve block, and the observation group were treated with spinal dorsal root pulse radiofrequency. Visual analogue score was used to evaluate the degree of pain before and 4 weeks after treatment. Self rating anxiety scale (SAS) and self rating Depression scale (SDS) were used to evaluate anxiety and depression, and to evaluate the clinical efficacy of the two groups. Results before treatment, the scores of VAS and SAS were compared between the two groups. After treatment, the scores of VAS and SAS in the two groups were lower than those before treatment, and the scores of VAS and SAS in the observation group were lower than those in the control group. The total effective rate of treatment in the observation group was 94.29, which was lower than that in the control group (77.14%, P 0.05). Conclusion the treatment of postherpetic neuralgia with epidural nerve block combined with pulse radiofrequency of dorsal root of spinal nerve can alleviate the pain of the patients. Indirectly ameliorating its bad emotion is helpful to improve the clinical curative effect.
