【摘要】:实验目的: 1.研究△Np63在正常皮肤、寻常型银屑病、脂溢性角化症、日光性角化病、基底细胞癌组织中的表达及意义。 2.探讨△Np63在皮肤良、恶性增殖性疾病中的诊断及鉴别诊断价值。 实验方法: 收集、整理近两年昆明医科大学第一附属医院皮肤科病理室的、符合诊断标准的寻常型银屑病(PS)、脂溢性角化症(SK)、日光性角化病(AK)、基底细胞癌(BCC)各30例,并选取皮肤科手术室及颌面外科手术切除的正常皮肤(Normal Skin)10例。经过脱水、包埋、切片等步骤,采用免疫组化技术SP法对切片进行△Np63染色,然后在光学显微镜下观察切片,每张切片在皮肤病变组织中随机选取5个不连续的视野,用OLYMPUS BX51显微图像采集系统拍摄图片,应用Image-Pro Plus6.0(IPP)图像分析软件定量测定每个视野区域内阳性表达的平均光密度值(density mean),最后通过spss17.0软件进行统计、分析病种之间的差异。阴性对照:PBS代替一抗,结果阴性。 实验结果: 1.△Np63在10例正常皮肤、30例寻常型银屑病、30例脂溢性角化症、30例日光性角化病、30例基底细胞癌中均有阳性表达。正常皮肤组,△Np63主要表达在表皮基底层细胞及近基底层细胞的细胞核中;寻常型银屑病及日光性角化病组,ANp63主要表达在表皮基底层及棘层中下1/3细胞的细胞核中;脂溢性角化症组,△Np63主要表达于表皮基底层及棘层细胞的细胞核中;基底细胞癌组,ANp63主要表达于基底样细胞组成的团块中呈弥漫的阳性表达。 2.经IPP软件统计,△Np63在各病种中的表达依次为:正常皮肤组的平均光密度值为0.0056±0.0019;日光性角化病组的平均光密度值为0.0262±0.0184;寻常型银屑病组的平均光密度值为0.0266±0.0100;脂溢性角化症组的平均光密度值为0.0391±0.0169;基底细胞癌组的平均光密度值为0.0498±0.0222。 3.经统计学分析,△Np63在各病种之间平均光密度的表达不同或不全相同(P0.001),差异有统计学意义;进一步进行两两比较,除日光性角化病及寻常型银屑病之间(P0.05)差异无统计学意义以外,其余各组之间两两比较(P0.05),差异均有统计学意义。其中△Np63在基底细胞癌组的表达最强,其次分别为脂溢性角化症组、寻常型银屑病组、日光性角化病组和正常皮肤组。 4.△Np63在各病种中的表达与患者的性别、年龄、发病部位(曝光部位和非曝光部位)均无统计学意义(P0.05)。 实验结论: 1.△Np63与复层鳞状上皮基底细胞的发育和增殖密切相关。 2.△Np63可作为是未分化上皮来源肿瘤的标记物。 3.△Np63可以考虑作为脂溢性角化症及日光性角化病鉴别诊断的免疫学指标。 4.△Np63蛋白的表达与患者的性别、年龄、日晒无相关性。
[Abstract]:Objective: 1. To study the expression and significance of Np63 in normal skin, psoriasis vulgaris, seborrheic keratosis, sunlight keratosis and basal cell carcinoma. 2. To investigate the value of Np63 in the diagnosis and differential diagnosis of benign and malignant proliferative diseases of skin. Experimental methods: In the past two years, 30 cases of psoriasis vulgaris, SKK, AKK, BCCs and BCCs were collected from the dermatology department of the first affiliated Hospital of Kunming Medical University, which met the diagnostic criteria. Normal Skin)10 cases of normal skin were selected from dermatological surgery room and maxillofacial surgery. After dehydration, embedding and slicing, immunohistochemical SP method was used to stain the sections with Np63, and then the sections were observed under optical microscope. Five discontinuous visual fields were randomly selected from each slice in the skin lesion tissue. OLYMPUS BX51 microscopic image acquisition system was used to take pictures, and Image-Pro plus 6.0 / IP) image analysis software was used to quantitatively measure the mean optical density of positive expression in each visual field. Finally, the difference of disease species was analyzed by spss17.0 software. The negative control group: PBS replaced the first antibody and the result was negative. Experimental results: 1. Np63 positive expression was found in 30 cases of seborrheic keratosis and 30 cases of basal cell carcinoma in 10 cases of normal skin and 30 cases of psoriasis vulgaris. In normal skin group, Np63 was mainly expressed in the nuclei of epidermal basal cells and proximal basal cells, while in psoriasis vulgaris and sunburn keratosis, ANp63 was mainly expressed in the nuclei of 1 / 3 cells in epidermal basal layer and lower layer of spinous layer. In seborrheic keratosis group, Np63 was mainly expressed in the nucleus of basal layer and spinous layer of epidermis, and ANp63 was expressed diffusely in the mass composed of basal cell in basal cell carcinoma group. 2. According to IPP software statistics, The average optical density of Np63 in normal skin group was 0.0056 卤0.0019, that of solar keratosis group was 0.0262 卤0.01844, that of psoriasis vulgaris group was 0.0266 卤0.0100, and that of seborrheic keratosis group was 0.0266 卤0.0100. The optical density was 0.0391 卤0.0169 and the average optical density in basal cell carcinoma was 0.0498 卤0.0222. 3. By statistical analysis, the average optical density of Np63 was different or not the same among the disease species, and the difference was statistically significant. There was no significant difference in P0.05 between solar keratosis and psoriasis vulgaris, but there was significant difference between the other two groups. The expression of Np63 was strongest in basal cell carcinoma group, followed by seborrheic keratosis group, psoriasis vulgaris group, sunlight keratosis group and normal skin group. 4. There was no significant difference between the expression of Np63 and the sex, age, location of the disease (exposure and non-exposure) of the patients (P 0.05). The experimental conclusions are as follows: 1. Np63 is closely related to the development and proliferation of basal cells in lamellar squamous epithelium. 2. Np63 can be used as a marker of undifferentiated epithelial-derived tumors. 3. Np63 may be considered as an immunological index for differential diagnosis of seborrheic keratosis and solar keratosis. 4. There was no correlation between the expression of Np63 protein and sex, age and sun exposure.
