寻常型银屑病患者血清中IL-17、IL-35的表达水平及其与PASI评分的相关性

【摘要】:目的检测寻常型银屑病患者血清中IL-17、IL-35的表达水平,分析它们在银屑病进行期和静止期的变化规律及其与PASI评分的相关性,进一步探讨它们在银屑病发病中的作用,为未来银屑病的治疗提供依据。方法1、研究对象:收集我院皮肤科门诊2016年2月至2016年11月收治的寻常型银屑病患者共29例。入选标准:(1)此类患者有明显的寻常型银屑病样皮损。(2)排除皮肌炎、天疱疮、慢性荨麻疹、恶性肿瘤、红斑狼疮等一系列自身免疫性疾病以及有其他合并症的患者。(3)该患者3个月内未使用过免疫抑制剂、维A酸类、类固醇激素等能影响自身免疫功能的药物。根据患者的病情将病例组分为两组:进行期组和静止期组。分别为16例和13例。由同一位医务人员对银屑病患者进行相应的PASI评分并做记录。健康对照组29例均来自我院体检中心的健康人员,且他们无自身免疫性疾病及相应的家族史,经t检验后,病例组和健康对照组在性别和年龄方面相比,差异无统计学意义。2、方法:采用双抗体夹心酶联免疫的方法(ELISA)对银屑病组和健康对照组人员血清中IL-17和IL-35的表达水平进行检测并做记录。3、统计:用SPSS 21.0软件对本实验所得的数据进行统计学处理。计量资料用均值±标准差(x±s)表示。计量资料两组间均数比较可用t检验;多组间则可用单因素方差分析(方差齐的用LSD法,方差不齐的用DunnettT3法)。分类变量则采用百分比表示,比较采用卡方检验。两个连续型随机变量的相关性用Pearson相关分析。P0.05为差异具有统计学意义。结果1、寻常型银屑病患者血清中IL-17表达水平高于健康对照组,差异有统计学意义(P0.05);其中,进行期血清中IL-17表达水平高于静止期,差异有统计学意义(P0.05)。2、寻常型银屑病患者血清中IL-35表达水平低于健康对照组,差异有统计学意义(P0.05);其中,进行期血清中IL-35的表达水平低于静止期,差异有统计学意义(P0.05)。3、寻常型银屑病患者血清中IL-17与IL-35表达水平呈负相关(P0.05)。4、寻常型银屑病患者血清中IL-17表达水平与PASI评分呈正相关(P0.05);寻常型银屑病患者血清中IL-35表达水平与PASI评分呈负相关(P0.05)。结论1、IL-17可能作为一种促炎因子参与了银屑病的发病;IL-35作为一种保护因子参与了银屑病的发病;且它们的表达水平可能与银屑病的严重程度有关。2、IL-17、IL-35失衡可能参与了银屑病的发病和免疫应答的过表达。
[Abstract]:Objective to detect the expression of IL-17 and IL-35 in serum of patients with psoriasis vulgaris, to analyze the changes of IL-17 IL-35 in psoriasis and its correlation with PASI score, and to explore the role of IL-17 IL-35 in the pathogenesis of psoriasis. To provide the basis for the treatment of psoriasis in the future. Methods 1. Participants: 29 patients with psoriasis vulgaris were collected from February 2016 to November 2016. Admission criteria: 1) these patients have obvious psoriasis vulgaris lesions.) exclude dermatomyositis, pemphigus, chronic urticaria, malignant tumor, A series of autoimmune diseases such as lupus erythematosus and other complications.) the patient has not used immunosuppressants, retinoic acids, steroids and other drugs that affect autoimmune function in 3 months. The patients were divided into two groups according to the patient's condition: the progressive group and the rest group. There were 16 cases and 13 cases respectively. The PASI score of psoriasis patients was recorded by the same medical staff. All 29 healthy persons in the healthy control group were from the physical examination center of our hospital, and they had no autoimmune disease and corresponding family history. After t test, the sex and age of the case group and the healthy control group were compared. Methods: the expression of IL-17 and IL-35 in the serum of psoriasis group and healthy control group were detected by double antibody sandwich enzyme-linked immunosorbent assay (Elisa) and recorded. 3. Statistics: SPSS 21.0 software was used to detect the expression of IL-35 and IL-17. The experimental data were processed statistically. The measurement data were expressed as mean 卤standard deviation (x 卤s). T test can be used to compare the mean of measurement data between two groups, and single factor analysis of variance (LSD method is used for homogeneous variance, DunnettT3 method is used for uneven variance). Classification variables were expressed as percentages, and chi-square tests were used to compare them. The correlation of two continuous random variables was statistically significant with Pearson correlation analysis. Results 1. The expression of IL-17 in patients with psoriasis vulgaris was higher than that in healthy controls (P 0.05), and the expression of IL-17 in patients with psoriasis vulgaris was higher than that in patients with psoriasis. The expression level of IL-35 in patients with psoriasis vulgaris was lower than that in healthy controls, and the difference was statistically significant (P 0.05), among which, the expression of IL-35 in the serum of patients with psoriasis vulgaris was lower than that in the stationary stage. The expression of IL-17 and IL-35 in patients with psoriasis vulgaris was negatively correlated with that in patients with psoriasis vulgaris. The expression of IL-17 in serum of patients with psoriasis vulgaris was positively correlated with the PASI score (P0.05), and the IL-35 table in serum of patients with psoriasis vulgaris was positively correlated with that of the patients with psoriasis vulgaris. There was a negative correlation between PASI score and reach level (P 0.05). Conclusion 1IL-17 may be a proinflammatory factor involved in the pathogenesis of psoriasis. IL-35, as a protective factor, may participate in the pathogenesis of psoriasis. Their expression level may be related to the severity of psoriasis. The imbalance of IL-17 and IL-35 may be involved in the pathogenesis of psoriasis and the overexpression of immune response.
