412例变态反应性疾病血清总IgE和特异性IgE检测结果分析

【摘要】: 目的 分析四种变态反应性疾病患者血清总IgE和过敏原特异性IgE (sIgE)检测结果,为更好地诊断、治疗和预防过敏性疾病提供可靠的科学依据和参考。 方法 选取2008年10月至2010年1月年中国医科大学附属第一医院皮肤科就诊的412例变态反应性疾病患者,入选患者分别诊断为湿疹、荨麻疹、特应性皮炎和过敏性鼻炎。采用免疫印迹定量过敏原检测系统对患者血清总IgE和过敏原sIgE进行检测,并采用统一设计的临床问卷,详细记录患者性别、年龄、发病时间、皮损形态及分布、用药史等情况,应用SPSS16.0软件包对不同疾病组血清总IgE和过敏原sIgE检测结果进行分析。 结果 检测412例变态反应性疾病患者和69例正常对照者的血清总IgE和过敏原sIgE,其结果如下: 1、412例变态反应疾病患者中,370例血清总IgE阳性,阳性率为89.8%;荨麻疹、湿疹、特应性皮炎及过敏性鼻炎这四种过敏性疾病总IgE阳性率不同,差异具有统计学意义(P0.05)。 2、412例变态反应疾病患者中,226例血清sIgE阳性,阳性率为54.9%。这四种变态反应性疾病sIgE阳性率高于正常对照组,差异具有统计学意义(P0.001)。其中,对一种过敏原sIgE反应阳性119例,以特应性皮炎最高,占同种疾病阳性反应的46.2%;对两种过敏原阳性反应62例,特应性皮炎最为明显,占同种疾病阳性反应的26.9%;对三种及三种以上过敏原检测阳性45例,以过敏性鼻炎为著,占同种疾病阳性反应的40%。 3、412例变态反应性疾病中,吸入性过敏原sIgE检出阳性率较食入性过敏原高,分别为65.3%和34.7%。其中,吸入组检出阳性率前4位分别是猫狗毛皮屑(16.3%)、户尘螨粉尘螨(13.4%)、蟑螂(10.1%)和多种霉菌(9.2%)。食入组中前3位分别是小麦(9.4%)、牛肉羊肉(8.1%)和鱼虾蟹(4.2%)。 4、慢性荨麻疹患者血清总IgE和过敏原sIgE同时阳性率高于急性荨麻疹,差异具有统计学意义(P0.001);急慢性荨麻疹患者中一种过敏原、两种过敏原和三种及三种以上过敏原sIgE检出阳性率不同,差异无统计学意义(P0.05);急慢性荨麻疹中不同种类过敏原sIgE的检测结果大致相同,但慢性荨麻疹患者中树木花粉、腰果花生黄豆和小麦sIgE阳性率较高,分别为7.2%、3.6%、7.8%。 结论 1、在荨麻疹、湿疹、特应性皮炎及过敏性鼻炎这四种变态反应性疾病中,总IgE检测阳性率不同,以荨麻疹最高,为93.3%。 2、荨麻疹、湿疹、特应性皮炎及过敏性鼻炎患者sIgE阳性率均高于正常对照组;其中吸入性过敏原sIgE阳性率高于食入性过敏原,吸入性过敏原主要为猫狗毛皮屑、户尘螨粉尘螨、蟑螂及多种霉菌,食入性过敏原主要为小麦、牛肉羊肉和鱼虾蟹。 3、与急性荨麻疹相比,慢性荨麻疹患者血清总IgE和过敏原sIgE同时阳性率较高,同时较易存在多种过敏原sIgE阳性;慢性荨麻疹患者中树木花粉、腰果花生黄豆和小麦sIgE阳性率高于急性荨麻疹。
[Abstract]:Objective to analyze the results of serum total IgE and allergen specific IgE (IgE) in patients with four allergic diseases so as to provide reliable scientific basis and reference for better diagnosis, treatment and prevention of allergic diseases. Methods 412 patients with allergic diseases were selected from the dermatology department of the first affiliated Hospital of China Medical University from October 2008 to January 2010. The patients were diagnosed as eczema and urticaria respectively. Atopic dermatitis and allergic rhinitis. Serum total IgE and allergen Siga E were detected by Western blot quantitative allergen detection system. The patients' gender, age, onset time, skin lesion morphology and distribution, drug history and so on were recorded by a unified clinical questionnaire. The results of serum total IgE and allergen sIgE in different disease groups were analyzed by SPSS 16.0 software package. Results Serum total IgE and allergen sIgE were measured in 412 patients with allergic diseases and 69 normal controls. The results were as follows: 1412 patients with allergic diseases were positive for serum total IgE. The total positive rate of IgE in urticaria, eczema, atopic dermatitis and allergic rhinitis was different, and the difference was statistically significant (P 0.05). The positive rate was 54.9%. The sIgE positive rate of these four allergic diseases was higher than that of the normal control group, and the difference was statistically significant (P 0.001). Among the 119 cases with positive Siga E reaction to one allergen, atopic dermatitis was the highest, accounting for 46.2% of the positive reaction for the same disease, while for 62 cases of the two allergens, atopic dermatitis was the most obvious, accounting for 26.9% of the positive reaction of the same disease. Among the 45 cases of allergic rhinitis detected by three or more allergens, it accounted for 40. 3412 cases of allergic diseases. The positive rate of Siga was higher than that of ingestion allergen, 65.3% and 34.7%, respectively. In the inhaled group, the first four positive rates were Cat and Dog hair dandruff (16.3C), Dust mites (13.4m), cockroach (10.1cm) and a variety of molds (9.2g). The positive rates of serum total IgE and allergen Siga in patients with chronic urticaria were significantly higher than those in patients with acute urticaria (P 0.001). The positive rates of one allergen, two allergens and three or more allergens in patients with acute and chronic urticaria were different, and the difference was not statistically significant (P 0.05). But the positive rates of tree pollen, cashew nut, soybean and wheat Siga were higher in patients with chronic urticaria. Conclusion 1. In urticaria, eczema, atopic dermatitis and allergic rhinitis, the positive rate of total IgE was different. The positive rate of Siga E in eczema, atopic dermatitis and allergic rhinitis was higher than that in normal control group, and the positive rate of inhaled allergen was higher than that of ingestion allergen. In cockroaches and many kinds of fungi, the main food allergens were wheat, beef and mutton, fish, shrimp and crab. 3. Compared with acute urticaria, the positive rates of serum total IgE and allergen Siga E in patients with chronic urticaria were higher than those in patients with chronic urticaria. The positive rate of tree pollen, cashew nut soybean and wheat Siga E in chronic urticaria was higher than that in acute urticaria.
