缺氧诱导因子-1α、葡萄糖转运蛋白-1、Ki-67在血管瘤和血管畸形中的表达及意义

【摘要】:目的检测缺氧诱导因子-1α(hypoxia-inducible factor 1 alpha,HIF-1α)、葡萄糖转运蛋白-1(glucose tracsporter-1,Glut-1)、Ki-67在不同时期血管瘤和血管畸形中的表达并探讨其意义。 方法选取广西医科大学第一附属医院病理科2005年1月~2009年12月手术切除的血管瘤归档标本45例,常规石蜡切片苏木精-伊红染色法(hematoxylin-eosin staining,HE)进行病理复检分组,另取10例正常儿童皮肤软组织作对照;所有标本做厚度4μm连续切片,常规脱蜡及水化,采用免疫组织化学(Immunohistochemistry,IHC)链霉菌抗生物素蛋白-过氧化物酶连结法(streptavidin-perosidase,SP)处理,高温高压修复抗原,每张切片于高倍镜下(400×)随机选取具有代表性的5个视野,通过计数阳性细胞个数来观察HIF-1α、Glut-1、Ki-67的表达。 结果45例血管瘤标本按Mulliken分类标准分为血管瘤30例,其中增生期19例,年龄3个月~1岁;消退期11例,年龄10个月~9岁;血管畸形15例,年龄7岁~52岁。HIF-1α阳性染色定位于血管内皮细胞的胞浆或胞核,呈棕黄色;增生期血管瘤内皮细胞的胞浆及胞核中有大量棕黄色颗粒,而消退期血管瘤、血管畸形及正常对照组只有少量棕黄色颗粒;统计结果表明增生期血管瘤中HIF-1α显著高于消退期血管瘤、血管畸形及正常对照组,差异有统计学意义(P0.05)。Glut-1阳性染色定位于血管内皮细胞的胞膜或胞浆,呈棕黄色;增生期血管瘤内皮细胞的胞膜中有大量棕黄色颗粒,消退期血管瘤内皮细胞的胞膜有少量棕黄色颗粒,而血管畸形及正常对照组中内皮细胞的胞膜只有极少量或没有棕黄色颗粒沉积;统计结果表明增生期血管瘤中Glut-1显著高于消退期血管瘤,差异有统计学意义(P0.05),血管畸形及正常对照组中Glut-1全为阴性表达。Ki-67阳性染色定位于血管内皮细胞的胞核,呈棕黄色或棕褐色;增生期血管瘤内皮细胞的胞核中有大量棕黄色或棕褐色颗粒,而消退期血管瘤、血管畸形及正常对照组的内皮细胞的胞浆及胞核中只有少量棕黄色颗粒,统计结果表明增生期血管瘤中Ki-67显著高于消退期血管瘤、血管畸形及正常对照组,差异有统计学意义(P0.05)。血管瘤中HIF-1α表达与Glut-1呈正相关(P0.05)。HIF-1α和Glut-1与Ki-67表达均呈正相关(P0.05)。 结论HIF-1α和Glut-1的异常表达与血管瘤的发生、发展有一定的关系,HIF-1α可能通过上调Glut-1的表达促进血管内皮细胞的增殖。
[Abstract]:Objective to detect the expression of hypoxia-inducible factor 1 alpha-HIF-1 伪 and glucose tracer 1 Glut-1 in different stages of hemangioma and vascular malformation. Methods from January 2005 to December 2009, 45 cases of hemangioma archived from Department of Pathology, first affiliated Hospital of Guangxi Medical University, were divided into two groups: routine paraffin section hematoxylin-eosin staining method (HE). The skin and soft tissue of 10 normal children were taken as control, all the specimens were made 4 渭 m thick serial sections, conventional dewaxing and hydration, and were treated with streptavidin-perosidase SP (streptavidin-perosidase SP) by Immunohistochemistry (IHC). The antigens were repaired at high temperature and high pressure. Five representative visual fields were randomly selected under high-power microscope (400 脳). The expression of HIF-1 伪 -Glut-1 Ki-67 was observed by counting the number of positive cells. Results according to Mulliken classification criteria, 45 hemangioma specimens were divided into 30 cases, including 19 cases in proliferative phase, 11 cases in regression phase, 10 months old and 9 years old, 15 cases in vascular malformation, 19 cases in proliferative phase, 1 year old in 3 months and 1 year old, 15 cases in vascular malformation. The positive staining of HIF-1 伪 was located in the cytoplasm or nucleus of vascular endothelial cells, and was brown in the cytoplasm and nucleus of proliferative hemangioma endothelial cells, while in the involuted phase, a large number of brown granules were found in the cytoplasm and nucleus of the endothelial cells of hemangioma. The statistical results showed that HIF-1 伪 in proliferative hemangioma was significantly higher than that in involutive hemangioma, vascular malformation and normal control group. The difference was statistically significant (P0.05). Glut-1 positive staining was located in the membrane or cytoplasm of vascular endothelial cells, and there were a lot of brown granules in the membranes of proliferative hemangioma endothelial cells. A small amount of brown granules were found in the membrane of endothelial cells in involutive phase, but only a few or no brown granules were found in the membrane of endothelial cells in vascular malformation and normal control group. The statistical results showed that Glut-1 in proliferative hemangioma was significantly higher than that in involutive hemangioma (P0.05). The expression of Glut-1 in vascular malformation and normal control group was all negative. Ki-67 positive staining was localized in the nucleus of vascular endothelial cells. A large number of brown or brown granules were found in the nucleus of endothelial cells in proliferative hemangioma, but only a few brown granules in the cytoplasm and nucleus of endothelial cells in involuted hemangioma, vascular malformation and normal control group. The statistical results showed that Ki-67 in proliferative hemangioma was significantly higher than that in involutive hemangioma, vascular malformation and normal control group (P0.05). HIF-1 伪 expression was positively correlated with Glut-1 expression (P0.05). HIF-1 伪 and Glut-1 expression were positively correlated with Ki-67 expression (P0.05). Conclusion the abnormal expression of HIF-1 伪 and Glut-1 is related to the occurrence and development of hemangioma. HIF-1 伪 may promote the proliferation of vascular endothelial cells by up-regulating the expression of Glut-1.
