RECK和MMP-2在皮肤鳞状细胞癌中的表达及临床意义

【摘要】:目的:观察基质金属蛋白酶抑制因子(RECK)和基质金属蛋白酶-2(MMP-2)在皮肤鳞状细胞癌(CSCC)、鲍温氏病、非典型增生组织中的表达,并探讨其与肿瘤发生、浸润转移的关系。 方法:采用免疫组化PV6000法检测55例(?)JCSCC组织(其中高分化27例、中分化18例、低分化10例;伴淋巴转移10例,远处转移2例)、13例鲍温氏病、24例不典型增生组织以及20例正常皮肤组织中的RECK和MMP-2蛋白表达,进而比较鳞状细胞癌在不同分化组、转移组、年龄及性别组中的表达情况,并将之结果进行统计学分析。 结果: 1镜下观察RECK的阳性表达主要位于正常鳞状上皮细胞及肿瘤细胞的细胞质中,呈浅黄色至棕黄色颗粒,部分胞核染色。MMP-2的阳性染色多定位于癌组织细胞质,呈浅黄色至棕黄色颗粒。 2 RECK在正常皮肤组织(CE)、非典型增生(AH)、鲍温氏病(BD)、皮肤鳞状细胞癌(CSCC)四种组织中的表达RECK在正常皮肤组织、非典型增生、鲍温氏病、皮肤鳞状细胞癌中的表达率分别为85.0%、54.2%、46.2%、43.6%,比较差异有统计学意义(P0.05)。 3 MMP-2蛋白正常皮肤组织(CE)、非典型增生(AH)、鲍温氏病(BD)、皮肤鳞状细胞癌(CSCC)四种组织中的表达MMP-2蛋白在正常皮肤组织、非典型增生、鲍温氏病、皮肤鳞状细胞癌中的表达率分别为25.0%、50.0%、53.8%、63.6%,比较差异有统计学意义(P0.05)。 4 RECK及MMP-2蛋白表达与皮肤鳞状细胞癌(CSCC)的组织学分级的关系根据组织学分级,皮肤鳞状细胞癌组织分为高中低分化三组。RECK阳性表达率随着肿瘤分化程度的降低而降低;而MMP-2阳性表达率随着肿瘤分化程度的降低而增高。差异均有统计学意义(P0.05)。 5 RECK及MMP-2蛋白与皮肤鳞状细胞癌(CSCC)淋巴转移和远处转移的关系无淋巴转移组中RECK阳性表达率明显高于淋巴转移组,比较差异有统计学意义(P0.01)。MMP-2在有淋巴转移的CSCC组中阳性表达率明显高于无淋巴转移组,两者表达差异有统计学意义(P0.01)。RECK (?)口MMP-2与鳞状细胞癌是否存在远处转移无关,表达无统计学意义(P0.05)。 6 RECK及MMP-2蛋白与皮肤鳞状细胞癌(CSCC)年龄和性别的关系RECK、MMP-2蛋白表达与皮肤鳞状细胞癌患者的年龄及性别无关,差异无统计学意义(P0.05)。 7 RECK与MMP-2在皮肤鳞状细胞癌(CSCC)中表达的关系RECK及MMP-2在皮肤鳞状细胞癌中的表达呈负相关。(r=-0.402 P0.01)。 结论: 1研究显示RECK在正常皮肤、非典型增生、鲍温氏病及鳞状细胞癌中均有表达,但表达水平依次减少,而MMP-2的表达水平依次增高,提示RECK的表达下调或缺失及MMP-2的过度表达在鳞状细胞癌的发生发展过程中可能起着重要作用。 2随着肿瘤组织学分化程度降低,RECK (?)日性率显著降低,MMP-2表达升高,提示RECK作为抑癌因子可能通过诱导细胞分化从而抑制鳞状细胞癌的发生,而MMP-2的上调表达则抑制了细胞分化过程,从而导致肿瘤恶性程度增高。两者可能为判断恶性程度提供依据。 3本研究发现RECK、MMP-2蛋白表达与淋巴结转移的发生显著相关,提示RECK表达降低,而MMP-2蛋白表达上调的肿瘤具有更强的侵蚀和转移能力。 4在鳞状细胞癌组织中RECK的低表达与MMP-2的表达升高有关。在皮肤鳞状细胞癌的发生发展过程中,两者呈显著的负相关。提示鳞状细胞癌组织中RECK表达的缺失导致了对MMP-2的抑制作用降低,而使MMP-2表达上调,进而促进了肿瘤细胞的侵袭和转移。
[Abstract]:Objective: To observe the expression of matrix metalloproteinase-2 (REK) and matrix metalloproteinase-2 (MMP-2) in skin squamous cell carcinoma (CSCC), Abalone's disease and atypical hyperplasia. Methods: 55 cases were detected by immunohistochemical PV6000 method. (?) JCSCC (in which 27 cases of high differentiation, 18 cases of medium differentiation, 10 with low differentiation, 10 cases with lymph node metastasis, 2 cases of distant metastasis), 13 cases of Bowen's disease, 24 cases of atypical hyperplasia and 20 normal skin tissues. The expression of squamous cell carcinoma in different group, transfer group, age and sex group was compared, and the results were statistically analyzed. Analysis. Results: The positive expression of RECK was mainly in the cytoplasm of normal squamous cell and tumor cells, and was pale yellow to brown yellow particles. The positive staining of MMP-2 was localized in the cytoplasm of the carcinoma and was pale yellow to the cytoplasm. The expression rate of REK in four tissues of normal skin tissue (CE), atypical hyperplasia (AH), Abalone's disease (BD) and skin squamous cell carcinoma (CSCC) was 85.0, respectively.%, 54.2%, 46.2%, 43.6%, and the difference was of statistical significance. The relationship between the age and sex of the skin squamous cell carcinoma (CSCC) was not related to the age and the sex of the skin squamous cell carcinoma (CSCC), and the expression of MMP-2 was not related to the age and sex of the skin squamous cell carcinoma. The relationship between the expression of MMP-2 in the skin squamous cell carcinoma (CSCC) and the expression of MMP-2 in the skin squamous cell carcinoma (CSCC) were not significant (P0.05). In the course of the development of squamous cell carcinoma of the skin, there was a significant negative correlation between the two groups. The deletion of the expression of the REK in the squamous cell carcinoma of the skin resulted in a decrease in the inhibition of MMP-2.
